Objective To analyze the clinical characteristics of chest tightness variant asthma(CTVA)in adult patients with chest tightness,in order to provide more ideas for the diagnosis of CTVA in the future.Methods A retrospective study was conducted on 289 patients with chest tightness who were admitted to the People's Hospital of Lishui City from January 2017 to December 2022.They were divided into the CTVA group of 57 patients and the non CTVA group of 232 patients.The differences in clinical characteristics such as gender,age,allergy history,clinical manifestations,laboratory indicators,and lung function indicators between the two groups were analyzed.A binary logistic regression model was used to screen relevant indicators for diagnostic thinking.Results The absolute value of eosinophils(EOS),serum immunoglobulin E(IgE),and exhaled nitric oxide(FeNO)concentration in the CTVA group were lower than those in the non CTVA group,and the specific IgE positive rate was lower than that in the non CTVA group.The proportion of smokers and the incidence of small airway function damage were higher than those in the non CTVA group,and the differences were statistically significant(P<0.05).The analysis results of the binary logistic regression model showed that absolute EOS values,IgE,specific IgE,FeNO,and small airway function impairment had certain discriminative ability for CTVA.Based on this model,the area under the curve of the column chart used for diagnosing CTVA is 0.879,and it has good calibration.Conclusion The levels of EOS,IgE,FeNO,and specific IgE positivity in peripheral blood of CTVA patients decrease,while the damage to small airway function increases.The use of these indicators can provide certain guidance for the diagnosis of CTVA in the future.
