Objective To investigate the risk factors and renal pathological characteristics in elderly patients with chronic glomerulonephritis accompanied by cardiovascular diseases(CVD).Methods Elderly patients who underwent renal biopsy and diagnosed with primary chronic glomerulonephritis in our hospital from January 2014 to December 2018 were enrolled.According to whether they were combined with cardiovascular disease or not,the patients were divided into the cardiovascular disease group(CVD group)and the non-cardiovascular disease group(no CVD group),the general data,clinical and pathological features of two groups of patients were reviewed.Results 1.A total of 254 patients were enrolled,including 97 patients in the CVD group.The most common renal histopathology in elderly patients with chronic glomerulonephritis was membranous nephropathy.2.The level of 2-hour postprandial blood glucose,hypersensitive C-reactive protein(hs-CRP),serum UA,serum cystatin C,parathyroid hormone,24-hour urine protein quantitation,alpha 1 microglobulin and blood calcium in the CVD group were significantly higher than those in the non-CVD group,and serum ALB,hemoglobin,e-GFR,and the level of urinary transferrin were lower than those in the non-CVD group(P<0.05).The renal pathological results showed that glomerular sclerosis and mesangial hypercellularity scores of the CVD group was higher than that of the non-CVD group(P<0.05).3.In univariate analysis,the risk factors associated with CVD were hypertension,serum albumin,hemoglobin,body mass index(BMI),blood calcium,hs-CRP.Further multivariate Logistic regression analysis showed that long history of hypertension,hypoalbuminemia,higher BMI and high hs-CRP levels were independent risk factors for elderly patients with chronic nephritis combined with CVD.Conclusion Elderly patients with chronic glomerulonephritis accompanied by CVD are prone to have metabolic-related diseases,higher proportion of glomerulosclerosis and higher mesangial proliferation score,suggesting more severe renal structural damage.Hypertension,hypoalbuminemia,high BMI,and high hs-CRP are independent risk factors for the occurrence of cardiovascular disease in elderly patients with chronic nephritis.
