Objective To analyze the Clinical features and prognostic factors of meningitis/meningoencephalitis caused by varicella zoster virus reactivation.Methods 74 patients were consistent with the criterion of VZV meningitis/meningoencephalitis in our center from July 2018 to July 2022 were included.The clinical presentations,including basic information,combined disease,herpes site,clinical symptoms,cerebrospinal fluid testing,imaging performance,time of herpes to neurological symptoms,starting time of drug treatment and outcome were retrospectively analyzed.Results Fever,headache,cranial nerve involvement/acute cognitive changes,meningeal irritation/Nausea and vomiting,Ramsay-Hunt syndrome(RHS)were the most common clinical manifestations of VZV meningitis/meningoencephalitis.There was no characteristic abnormal manifestation on brain MRI.Compared with VZV meningoencephalitis,the age of patients with VZV meningitis was younger(P=0.01),had lower cerebrospinal fluid(CSF)glucose content(P=0.02),and had better outcome at discharge(P<0.001).Male,the days between beginning neurological symptoms to antiviral treatment longer than 1.5 days were the independent risk factor for the outcome of varicella-zoster virus meningitis/meningoencephalitis at discharge.Conclusion Early intravenous antiviral treatment for VZV meningitis/meningoencephalitis mostly has a good outcome.Male and the days between beginning neurological symptoms to antiviral treatment longer than 1.5days,the worse the outcomes.
