目的 分析成人尿邻苯二甲酸盐代谢物与高血压的相关性.方法 从2003-2012年美国国家健康和营养检查调查(NHANES)中提取包括八种尿液邻苯二甲酸酯代谢物的水平,以及邻苯二甲酸2-乙基已酯代谢产物(ΣDEHP)的汇总.根据2017年美国心脏病学会/美国心脏协会指南,高血压被定义为收缩压≥130 mm Hg或舒张压≥80 mm Hg.在控制年龄、性别、种族/民族、教育水平、尿肌酐、体质量指数和糖尿病后,通过二元逻辑回归模型估计邻苯二甲酸酯代谢物与高血压的比值比(OR).进行受试者操作特征(ROC)曲线分析,以检验ΣDEHP对高血压的预测.结果 ΣDEHP高的参与者有更高的高血压患病率.二元Logistic逻辑回归显示,ΣDEHP水平与高血压存在相关性(P<0.05).ROC分析显示,ΣDEHP可以预测整个人群以及男性和女性人群的高血压.结论 ΣDEHP与成人高血压患病率密切相关.
Objective To evaluate the association of urinary phthalate metabolites with hypertension in adults.Methods The data were extracted from National Health and Nutrition Examination Survey(NHANES)2003-2012 with levels of eight urinary phthalate metabolites,as well as a summary di-2-ethylhexyl phthalate metabolites(ΣDEHP).According to 2017 the American College of Cardiology/American Heart Association guideline,hypertension was defined as systolic blood pressure≥130 mmHg or diastolic blood pressure≥80 mmHg.Odds ratios(ORs)of quartiles of phthalate metabolites for hypertension by the binary logistic regression models after controlling for age,gender,race/ethnicity,education level,urinary creatinine,body mass index,diabetes were estimated.Receiver-operating characteristic(ROC)analyses were conducted to examine the prediction of ΣDEHP for hypertension.Results Most of higher levels of phthalate metabolites has higher prevalence of hypertension.The Logistic regression indicated that all quartiles of ΣDEHP were associated with hypertension(P<0.05).ROC analysis showed that ΣDEHP predicted hypertension in entire population.Conclusion The study presents close relationship between ΣDEHP and hypertension prevalence in American adults.
