Objective To explore the efficacy and safety of domestic disposable digital flexible ureteroscope in the treatment of upper urinary tract stones with a major diameter>2 cm.Methods A retrospective analysis was performed on the clinical data of 39 patients who had upper urinary tract stones with a major diameters>2 cm treated with domestic disposable electronic ureteroscopes in our hospital from January 2021 to May 2023.There were 30 males and 9 females.The average age was 51.6(31~73)years.The average sum of the major diameters of the stones was 2.8(2.1~6.9)cm,and the average CT value was 1005.2(577~1361)HU.In 28 cases,a ureteral stent was left in place for 1-2 weeks before surgery,and 11 cases underwent one-stage surgery.In all 39 cases,an F14/12 ureteral guide sheath was successfully inserted during surgery,and no stone basket was used.The number of surgeries,laser lithotripsy time,postoperative fever,ureteral stone street,and stone-free rate were recorded.Results A total of 30 cases(76.9%)underwent a single operation,7 cases(18.0%)underwent two operations,and 2 cases(5.1%)underwent three operations.The average stone length in patients undergoing multiple operations was 3.7 cm(ranging from 2.1 to 6.9 cm),which was significantly longer than the 2.4 cm(ranging from 2.1 to 3.7 cm)in those undergoing a single operation(P<0.001).The average CT value of 1164.9 HU(ranging from 1062 to 1282 HU)in patients undergoing multiple operations was also significantly higher than the 975.9 HU(ranging from 532 to 1361 HU)in those undergoing a single operation(P=0.019).The average laser lithotripsy time was 65.3 min(ranging from 30 to 120 min).Three months after the operation,the SFR was 64.1%(25 of the 39 cases).In the 20 cases with stones in the lower calyx,the SFR was 65%(13 of 20 cases).The average length of the remaining stones was 0.73 cm(ranging from 0.5 to 1.2 cm).3 cases of postoperative fever,1 case of lower ureteral stone street.No urinary sepsis,subcapsular renal hematoma,ureteral perforation,tear,stricture and other complications.50 operations were performed without any damage to the disposable digital flexible ureteroscope.Conclusion The use of domestic disposable digital flexible ureteroscope for the treatment of upper urinary tract stones with a major diameter of>2 cm is safe and effective,and is worthy of promotion and application.
