Effect of'Four Gates-Three Winds'Needling on Serum Expressions of BDNF,SP,DA and 5-HT and NGF in Treatment of Post-Stroke Dysphagia of Acute Phase
Objective:To explore the clinical efficacy of'four gates-three winds'needling in treatment of post-stroke dysphagia of acute phase,and to analyze its effect on serum levels of brain-derived neurotrophic factor(BDNF),substance P(SP),dopamine(DA),5-hydroxytryptamine(5-HT)and nerve growth factor(NGF).Methods:100 patients with post-stroke dysphagia of acute phase were randomly assigned to the control group and the treatment group,with 50 patients in each group.The conventional treatment was applied to both groups,patients in the control group also received swallowing function training;on which basis,the treatment group was also treated with'four gates-three winds'needling,once per day,6 d was taken as one course of treatment.Both groups were treated for 2 courses.Efficacy was evaluated after the treatment.The swallowing function were evaluated by the standard swallowing assessment(SSA)and the Modified Mann Assessment of Swallowing Ability(MMASA).The serum expressions of BDNF,SP,DA,5-HT and NGF were analyzed in the two groups.SSA scale was applied to the follow-up at 2 months of stroke.Results:After 2 weeks of treatment,SSA score and MMASA score were significantly improved compared with those before the treatment in the two groups(P<0.05),SSA score was lower in the treatment group than that in the control group(P<0.05),and MMASA score was higher in the treatment group than that in the control group(P<0.05).The total effective rate was 92.0%(40/50)in the treatment group,which higher than 80.0%(40/50)in the control group(P<0.05).The expressions of BDNF,SP,DA,5-HT and NGF were higher after the treatment than those before the treatment in the two groups(P<0.05),which were higher in the treatment group than those in the control group after the treatment(P<0.05).The SSA score showed no significant difference at two months after stroke compared with that after the treatment(P>0.05);however,the SSA score was lower in the treatment group than that in control group(P<0.05).Conclusion:'Four gates-three winds'needling is effective in treating post-stroke dysphagia of acute phase,which can significantly improve the swallowing function of the patients and has a long-term efficacy.The mechanism may be related to the improvement of the serum expressions of BDNF,SP,DA,5-HT and NGF.
