Effect of Acupotomy Injection on Pain Degree and Inflammatory Reaction in Patients with Discogenic Low Back Pain
Objective:To observe the therapeutic effect of acupotomy injection on patients with discogenic low back pain.Methods:90 cases of discogenic low back pain were randomly divided into the acupotomy injection group,the injection group and the acupotomy group,with 30 cases in each group.The injection group was treated with local injection around pain points,the acupotomy group was treated with conventional acupotomy,and the acupotomy injection group was treated with acupotomy combined with injection.Clinical efficacy was evaluated after one month of treatment in the three groups.The improvements on local pain and overall symptoms were assessed by observing the score changes of VAS,JO A and ODI in the three groups after the treatment.The depression state of the three groups was observed with Self Rating Depression Scale(SDS).The serum levels of hexaketoprostaglandin F1α(PGF1 α),tumor necrosis factor α(TNF-α),interleukin-1 β(IL-1 β)and interleukin-6(IL-6)were detected before and after the treatment in the three groups.Results:There were no statistical differences in the indicators among the three groups before the treatment(P>0.05).After the treatment,the clinical efficacy of the acupotomy injection group was better than that of the injection group and the acupotomy group(P<0.05).Compared with those in the injection group and the acupotomy group,the scores of VAS,ODI and SDS decreased significantly,whereas the score of JO A increased significantly in the acupotomy injection group(P<0.05).The serum levels of PGF1 α,TNF-α,IL-1 β and IL-6 were significantly lower in the acupotomy injection group than those of the injection group and the acupotomy group(P<0.05).The scores of VAS,ODI and SDS,the serum levels of PGF1 α,TNF-α,IL-1β and IL-6 were significantly lower,and the score of JOA was obviously higher in the injection group than those in the acupotomy group(P<0.05).Conclusion:Acupotomy injection has a good therapeutic effect on discogenic low back pain,which is superior to local injection and simple acupotomy treatment.Its mechanism may be related to reducing the level of inflammatory cytokines and inhibiting inflammatory reaction.
