Clinical Study of Ulnar Tibial Needling Combined with Danlu Tongdu Tablet and McKenzie Therapy in Treatment of LDH Complicated with Cauda Equina Nerve Injury
Objective:To investigate the efficacy and analgesic effect of ulnar tibia needling combined with Danlu Tongdu Tablet and McKenzie therapy in treatment of lumbar disc herniation(LDH)complicated with cauda equina nerve injury,and to explore its influence to the pain-inducing factors of ERK and p-P38.Methods:90 patients with lumbar disc herniation complicated with cauda equina nerve injury were randomly divided into the control group and the study group,with 45 cases in each group.The control group was treated with Danlu Tongdu Tablet combined with McKenzie therapy;on which basis,the study group was also treated with ulnar tibia needling.Therapeutic effect was evaluated after 4 weeks of continuous treatment.Frankel spinal cord injury score,VAS pain score,serum prostaglandin E2(PGE2),5-hydroxytryptamine(5-HT),nitric oxide(NO)and other pain-inducing factors,as well as the expressions of serum extracellular regulatory protein kinase(ERK)and phosphorylated-P38 mitogen activated protein kinase(p-P38MAPK)were compared between before and after the treatment in the two groups.Results:The total effective rate was 91.11％(41/45)in the study group,which was significantly higher than 73.33％(33/45)in the control group(P＜0.05).The scores for lower limb radiation pain,numbness and weakness,difficulty in bowel movement,and difficulty in urinating were lower in the study group than those in the control group after the treatment(P＜0.05).The Frankel spinal cord injury score was higher(P＜0.05),and the VAS score was lower in the study group than that in the control group after the treatment(P＜0.05).The serum levels of PGE2,5-HT and NO were lower in the study group than those in the control group after the treatment(P＜0.05).The expressions of serum ERK and p-P38MAPK were lower in the study group than those in the control group after the treatment(P＜0.05).Conclusion:The combination of Danlu Tongdu Tablet and ulnar tibial needling combined with Mackenzie therapy has a better therapeutic effect on patients with lumbar disc herniation complicated with cauda equina nerve injury.It helps to repair the cauda equina nerve injury,reduce the expressions of pain-inducing factors,thus to alleviate the pain symptoms of lower back and leg.The possible mechanism is related to affecting the expressions of ERK and p-P38MAPK.
