针灸治疗卒中后失眠的作用机制研究进展

针灸是治疗卒中后失眠的有效方式之一,但其作用机制却鲜有阐述.本研究梳理了近年来国内外针灸治疗卒中后失眠的机制研究文献,结果显示,针灸通过干预相关神经递质及细胞因子的含量,改善睡眠结构,增加REM睡眠时间来提升睡眠质量,缓解失眠症状;针灸还可调控人体昼夜节律及生物钟功能,进一步协调睡眠-觉醒周期,恢复正常睡眠;此外,针灸调节脑血管反应性,增加局部脑血流供应,有效治疗PSI.未来研究应从对单一因素的研究逐渐转向多个靶点间相互作用的探索,进一步探究针灸治疗的优势选穴、刺激参数,不同证型的治疗方案等,多角度、多层次探明针灸治疗PSI的相关机制.
Research Advance on Mechanism of Acupuncture and Moxibustion in Treating Post-Stroke Insomnia
Acupuncture and moxibustion is one of the effective modalities for the treatment of post-stroke insomnia,but its action mechanism has rarely been elaborated.This study summarized the research literature on the mechanism of acupuncture and moxibustion in the treatment of post-stroke insomnia at home and abroad in recent years.It is found that acupuncture and moxibustion can improve sleep quality and alleviate insomnia symptoms by interfering with the content of related neurotransmitters and cytokines,improving sleep structure,and increasing REM sleep time.Acupuncture and moxibustion can also regulate human circadian rhythm and biological clock function to further harmonize the sleep-wake cycle and restore normal sleep.In addition,acupuncture and moxibustion regulates cerebrovascular reactivity and increases local cerebral blood flow supply to treat post-stroke insomnia effectively.Future researches should gradually shift from the study of a single factor to the exploration of interactions between multiple targets.It is suggested to further explore the advantageous selection of acupuncture points,stimulation parameters,and treatment protocols for different types of evidence,so as to explore the mechanisms related to the treatment of post-stroke insomnia with acupuncture and moxibustion from multiple perspectives and at multiple levels.
