Factors affecting blood pressure control in patients with H-type hypertension in a tertiary hospital in Xinjiang
Objective To determine the blood pressure control and its influencing factors in patients with H-type hypertension who regularly take antihypertensive medication according to guidelines,to provide a scientific basis for the prevention and treatment of H-type hypertension.Methods Totally 728 inpatients with H-type hypertension who met the inclusion and exclusion criteria were retrospectively collected from the First Affiliated Hospital of Xinjiang Medical University between January 2019 and December 2022.We collected basic patient information,laboratory results,medication and disease information,and follow-up data by reviewing medical records and conducting telephone follow-ups.The patients were divided into two groups based on their blood pressure after being discharged from the hospital and taking regular antihypertensive medication for at least one month:the uncontrolled group(276 patients)and the controlled group(452 patients).Univariate and Logistic regression analyses were used to determine the factors associated with the effectiveness of blood pressure control.Results The univariate analysis indicated significant differences between the two groups in hypertension grading,antihypertension drug combination,history of calcium channel blocker medication,mental stress condition,dietary habits,and frequency of exercise(P<0.05).The multi-factor analysis showed that hypertension grade Ⅲ,TT type in the MTHFR C677T gene polymorphism,frequent mental stress,salt intake habits,and lack of exercise were independent risk factors for blood pressure control in patients with H-type hypertension(P<0.05).Conclusion Factors affecting the level of blood pressure control in H-type hypertension patients taking regular antihypertension drugs include hypertension classification,MTHFR C677T genotype,mental stress condition,dietary habit and frequency of exercise.Effective interventions can be taken according to the relevant influencing factors to better control the blood pressure in H-type hypertension patients.
