Development and application of medication compliance scale in patients with chronic obstructive pulmonary disease
Objective To develop a medication compliance scale for patients with chronic obstructive pulmonary disease(COPD)based on the theory of planned behaviors and evaluate its reliability and validity.Methods The scale was constructed through literature review,semi-structured interviews,Delphi expert consultation and clinical investigation.Statistical methods such as item analysis,Cronbach's α coefficient method,and factor analysis were used to test the reliability and validity of the scale.Multiple linear regression analysis was used to determine the influencing factors of medication adherence.Results Structure validity of the scale showed:c2/df=2.971,GFI=0.916,RMSEA=0.075,RMA=0.097,CFI=0.926,NFI=0.911,IFI=0.932.The scale S-CVI/Ave was 0.98,the I-CVI of each item was 0.90~1.00;the Cronbach's α coefficient of the scale was 0.798 and each dimension was 0.788,0.760,0.665,and 0.767;the split-half reliability Spearman-Brown coefficient was 0.574.The total score of medication adherence in patients with COPD was(48.82±8.51).Education level,duration of COPD diagnosis,household income,and current residence were the influencing factors of medication compliance(P<0.05).Conclusion The scale developed in this study has good reliability and validity,which can be used for the evaluation of medication adherence in patients with COPD.Medication adherence among patients with COPD is at a moderate level,with low scores subjective norms and perceived behavioral control.Healthcare workers should pay special attention to patients with a short course of diagnosis,low education levels,and low family income,and living in rural areas or towns,strengthen health education for vulnerable groups,combine family and social support to improve medication compliance to ensure treatment effectiveness.
