生物反馈电刺激治疗压力性尿失禁的效果

目的:分析生物反馈电刺激治疗压力性尿失禁(SUI)的效果。方法:选取 2019 年 11 月—2022 年 11 月无锡市康复医院收治的 112 例SUI患者作为研究对象。根据随机数表法将其分为对照组和观察组,各 56 例。对照组给予常规治疗,观察组在对照组基础上给予生物反馈电刺激。比较两组治疗前后漏尿情况、盆底肌肌力及肌纤维平均肌电值、尿流动力学参数。结果:治疗后,两组排尿次数、漏尿次数及 1 h尿垫试验漏尿量均下降,观察组排尿次数、漏尿次数及 1 h尿垫试验漏尿量均明显低于对照组,差异有统计学意义(P<0。05)。治疗后,两组Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力及肌纤维平均肌电值均明显升高,观察组Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力及肌纤维平均肌电值均明显高于对照组,差异有统计学意义(P<0。05)。治疗后,两组腹压漏尿点压(ALPP)、功能性尿道长度(SFL)、最大尿道压(MUP)、最大尿道闭合压(MUCP)均明显升高,观察组ALPP、SFL、MUP、MUCP均明显高于对照组,差异有统计学意义(P<0。05)。结论:生物反馈电刺激治疗SUI可改善漏尿情况,提升盆底肌力,改善尿动力学指标。
Effect of Biofeedback Electrical Stimulation in the Treatment of Stress Urinary Incontinence
Objective:To analyze the effect of biofeedback electrical stimulation in the treatment of stress urinary incontinence(SUI).Method:A total of 112 SUI patients admitted to Wuxi Rehabilitation Hospital from November 2019 to November 2022 were selected as the study objects.They were divided into control group and observation group according to random number table method,with 56 cases in each group.The control group was given conventional treatment,and the observation group was given biofeedback electrical stimulation based on the control group.Urine leakage,pelvic floor muscle strength,average electromyography value of muscle fibers and urodynamic parameters before and after treatment were compared between the two groups.Result:After treatment,the frequency of urination,the frequency of urine leakage and the 1 h urine pad test urine leakage in both groups decreased,and the frequency of urination,the frequency of urine leakage and the 1 h urine pad test urine leakage in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the muscle strength of class Ⅰ muscle fiber,the muscle strength of class Ⅱ muscle fiber and the average electromyography value of muscle fiber were significantly increased in two groups,and the muscle strength of class Ⅰmuscle fiber,the muscle strength of class Ⅱ muscle fiber and the average electromyography value of muscle fiber in observation group were significantly higher than those in control group,the differences were statistically significant(P<0.05).After treatment,abdominal leak point pressure(ALPP),functional urethral length(SFL),maximum urethral pressure(MUP),maximum urethral closure pressure(MUCP)in both groups were significantly increased,and ALPP,SFL,MUP and MUCP in observation group were significantly higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:Biofeedback electrical stimulation for SUI can improve urinary leakage condition,enhance pelvic floor muscle strength,and improve urodynamic indexes.
