Application of Combined Epidural Anesthesia in Elderly Patients with Hip Fracture Surgery
Objective:To explore the application of combined epidural anesthesia in elderly patients with hip fracture surgery.Method:A total of 80 elderly patients with hip fracture surgery admitted to the Anesthesiology Department of People's Hospital of Xinhui District,Jiangmen City from November 2020 to November 2021 were selected as the study objects.According to the random method,they were divided into the conventional group and the experimental group,with 40 cases in each group.The conventional group was given continuous epidural anesthesia,and the experimental group was given combined epidural anesthesia.The heart rate,systolic blood pressure and diastolic blood pressure at the time of entry(T0),10 min after the operation(T1),30 min after the operation(T2)and at the end of the operation(T3)of the two groups were compared.The stress indexes before surgery and 12 h after surgery,anesthetic effect,anesthetic excellent and good rate,pain stuation at 10 min after surgery,1 h,3 h,6 h,12 h after surgery,and adverse reactions were compared between the two groups.Result:T1,heart rate and diastolic blood pressure of experimental group were higher than those of conventional group,T2,T3,heart rate,diastolic blood pressure and systolic blood pressure of experimental group were higher than those of conventional group,the differences were statistically significant(P<0.05).At 12 h after operation,the levels of adrenaline(AD),norepinephrine(NE)and C-reactive protein(CRP)in experimental group were significantly lower than those in conventional group,the differences were statistically significant(P<0.05).The onset time of anesthesia and perfect time of sensory block in experimental group were shorter than those in conventional group,the differences were statistically significant(P<0.05).The excellent and good rate of experimental group was higher than that of conventional group,the difference was statistically significant(P<0.05).At 1 h,3 h,6 h and 12 h after operation,the visual analogue scale(VAS)score of experimental group were lower than those of conventional group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in experimental group was lower than that in conventional group,the difference was statistically significant(P<0.05).Conclusion:The combined epidural anesthesia in elderly patients undergoing hip fracture surgery has short onset time and good analgesic effect,which can reduce the stress response and postoperative pain of patients without increasing the adverse reactions related to anesthesia,and can achieve safe and efficient anesthetic effects.
