Effect of Three-hole Laparoscopic Surgery and Open Surgery in the Treatment of Patients with Acute Appendicitis
Objective:To compare the effect of three-hole laparoscopic surgery and open surgery in the treatment of patients with acute appendicitis.Method:The clinical data of 105 patients with acute appendicitis admitted to Youxi General Hospital from January 2020 to January 2022 were retrospectively analyzed. They were divided into laparotomy group(n=49)and laparoscopic group(n=56)according to different surgical methods. The laparotomy group was given open abdomen surgery, and the laparoscopic group was given three-hole laparoscopic surgery.Perioperative indexes, gastrointestinal function recovery condition, stress and inflammation indexes before and 3 d after surgery and complications were compared between the two groups.Result:The surgery time and hospital stay in the laparoscopic group were significantly shorter than those in the laparotomy group, the first time of getting out of bed was earlier than that in the laparotomy group, and the intraoperative blood loss volume was significantly less than that in the laparotomy group, the differences were statistically significant(P<0.05).The bowel sound recovery time, first exhaust time and feeding time in laparoscopic group were significantly earlier than those in laparotomy group, and the differences were statistically significant(P<0.05).At 3 d after surgery, malondialdehyde(MDA)and interleukin-6(IL-6)levels increased in both groups, and the levels of MDA and IL-6 in laparoscopic group were significantly lower than those in laparotomy group, and the differences were statistically significant(P<0.05).The incidence of complications in laparoscopic group was significantly lower than that in laparotomy group, and the difference was statistically significant(P<0.05).Conclusion:Three-hole laparoscopic surgery for acute appendicitis can shorten operation time, reduce intraoperative bleeding, promote postoperative gastrointestinal function recovery of patients, reduce oxidative stress damage, and reduce the incidence of complications.
