摘要
目的:探讨颈动脉超声参数联合心血管健康评分对高血压心血管病变的诊断价值.方法:选取 2020 年 1 月—2022 年 1 月首都医科大学怀柔教学医院收治的 80 例高血压患者.根据是否存在心血管病变将其分为单纯高血压组(44 例)与心血管病变组(36 例).两组均进行颈动脉超声检查、超声心动图检查及心血管健康评分,并收集两组一般资料.对高血压心血管病变进行单因素及多因素分析,分析颈动脉超声参数联合心血管健康评分对高血压心血管病变的诊断价值.结果:心血管病变组Crouse斑块积分、斑块数目、cIMT、FRS评分、危险概率>10%占比、吸烟史占比均高于单纯高血压组,差异有统计学意义(P<0.05).logistic结果显示,Crouse斑块积分、斑块数目、cIMT、FRS评分均是高血压心血管病变的独立危险因素(P<0.05).受试者工作特征(ROC)曲线显示,Crouse斑块积分、斑块数目、cIMT、FRS评分、联合检测诊断高血压心血管病变的曲线下面积分别为 0.813、0.732、0.805、0.734、0.935.结论:颈动脉超声参数联合心血管健康评分对高血压心血管病变具有较高的诊断价值.
Abstract
Objective:To explore the diagnostic value of carotid artery ultrasound parameters combined with cardiovascular health score on hypertensive cardiovascular disease.Method:A total of 80 hypertensive patients admitted to Huairou Teaching Hospital,Capital Medical University from January 2020 to January 2022 were selected.According to the presence or absence of cardiovascular disease,the patients were divided into simple hypertension group(44 cases)and cardiovascular disease group(36 cases).Carotid artery ultrasound,echocardiography and cardiovascular health scores were performed in both groups,and general data of both groups were collected.Univariate and multivariate analysis of hypertensive cardiovascular disease was performed,the diagnostic value of carotid artery ultrasound parameters combined with cardiovascular health score in hypertensive cardiovascular diseases was analyzed.Result:The Crouse plaque score,plaque number,cIMT,FRS scores,risk probability>10%proportion and smoking history proportion of the cardiovascular disease group were all higher than those of the simple hypertension group,the differences were statistically significant(P<0.05).logistic results showed that crouse plaque score,plaque number,cIMT and FRS scores were independent risk factors for hypertensive cardiovascular disease(P<0.05).Receiver operating characteristic(ROC)curve showed that the area under the curve of crouse plaque score,plaque number,cIMT,FRS score and combined test for diagnosis of hypertensive cardiovascular disease were 0.813,0.732,0.805,0.734 and 0.935 respectively.Conclusion:Carotid artery ultrasound parameters combined with cardiovascular health scores have high diagnostic value for hypertensive cardiovascular diseases.
