摘要
目的:探讨阶段性营养干预联合心理护理对造血干细胞移植(hematopoietic stem cell transplantation,HSCT)患者心理状态、营养状态、生活质量的影响.方法:选取 2020 年 1 月—2022 年 12 月在徐州医科大学附属医院血液科进行HSCT的 113 例患者作为研究对象,随机分为观察组 58 例和对照组 55 例.对照组给予常规护理干预,观察组在常规护理的基础上给予阶段性营养干预联合心理护理.比较两组干预前后患者的心理状态、营养状况、生活质量.结果:干预后,两组症状自评量表(symptom check list 90,SCL-90)各维度评分均较干预前显著降低,观察组显著低于对照组,差异有统计学意义(P<0.05).干预后,两组前白蛋白、白蛋白均较干预前显著降低,观察组高于对照组,差异有统计学意义(P<0.05).干预后,两组功能领域、总体健康状况评分均较干预前显著降低,观察组高于对照组,症状领域评分均较干预前显著上升,观察组低于对照组,差异有统计学意义(P<0.05).结论:在HSCT的不同阶段给予相应的营养干预及心理护理,能明显缓解患者的心理压力,改善营养状态,提升生活质量.
Abstract
Objective:To explore the effects of staged nutrition intervention combined with psychological nursing on the psychological status,nutritional status and quality of life of patients with hematopoietic stem cell transplantation(HSCT).Method:A total of 113 patients undergoing HSCT in the Department of Hematology,the Affiliated Hospital of Xuzhou Medical Unversity from January 2020 to December 2022 were selected as the study objects,and the were randomly divided into observation group 58 cases and control group 55 cases.The control group was given routine nursing intervention,and the observation group was given staged nutrition intervention combined with psychological nursing on the basis of routine nursing.The psychological state,nutritional status and quality of life of the two groups before and after intervention were compared.Result:After intervention,symptom check list 90(SCL-90)scores of both groups were significantly lower than before intervention,and the observation group were significantly lower than the control group,the differences were statistically significant(P<0.05).After intervention,prealbumin and albumin in both groups were significantly lower than before intervention,and the observation group were higher than the control group,the differences were statistically significant(P<0.05).After intervention,the scores of functional areas and general health status of both groups were significantly lower than before intervention,the observation group were higher than the control group,and the scores of symptom areas were significantly higher than before intervention,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).Conclusion:Nutritional intervention and psychological nursing at different stages of HSCT can obviously relieve the psychological pressure of patients,improve the nutritional status and improve the quality of life.
