Correlation Analysis of Sperm DNA Damage with Semen Parameters,Embryonic Development and Recurrent Spontaneous Abortion
Objective:To observe the correlation between sperm DNA damage and semen parameters,embryonic development and recurrent spontaneous abortion(RSA).Method:A total of 300 patients with sperm DNA damage who had fertility requirements in Gaoming District People's Hospital in Foshan City from January 2019 to March 2022 were selected as the observation group,and 50 males with normal sperm morphology(sperm normal morphology ratio≥4%)whose spouses were RSA patients in the same period were selected as the abortion group,and 50 healthy males with fertility requirements were selected as the control group.The semen parameters(sperm density,sperm motility,and sperm morphology)among three groups were compared,according to the sperm DNA fragmentation index,the observation group patients were divided into the high damage group(sperm DNA fragmentation index≥30%,n=127)and the low damage group(sperm DNA fragmentation index<30%,n=173),embryonic development and recurrence of RSA between the spouses of two subgroups of patients(successful pregnancy after natural pregnancy and using assisted reproductive technology)were followed up and compared,the relationship between sperm DNA damage and sperm density,sperm motility,sperm morphology,patients'spouse's embryonic development,RSA were analyzed.Result:There were significant differences in semen parameters among three groups(P<0.05);the sperm density,sperm motility and sperm morphology of the observation group were significantly lower than those of the abortion group and the control group,which of the abortion group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).The incidence of embryonic development arrest and RSA in the naturally successful pregnancy spouse in the high damage group after treatment were significantly higher than those in the low damage group,the differences were statistically significant(P<0.05);the success rate of embryo development arrest during pregnancy using assisted reproductive technology in the high damage group was 6.97%,and the incidence of RSA was 11.63%.The sperm DNA fragment index were negatively correlated with sperm density,sperm motility and sperm morphology(P<0.05),and positively correlated with the stagnation of embryonic development and RSA after the natural pregnancy of the patients'spouse(P<0.05).Conclusion:There are certain correlation between sperm DNA damage and semen parameters,embryonic development and RSA,the higher the degree of DNA damage to sperm,the poorer the quality of their semen,and the higher the likelihood of embryonic development stagnation and RSA occurrence after natural pregnancy,clinical evaluation of infertility can be based on the patients'sperm DNA damage,providing a reference basis for clinical pregnancy selection.
