基于循证理论的护理在重症心力衰竭无创呼吸机治疗患者中的应用

目的:探讨基于循证理论的护理在重症心力衰竭无创呼吸机治疗患者中的应用。方法:选取 2021 年 2 月—2023 年 2 月福建医科大学附属第一医院收治的 82 例重症心力衰竭无创呼吸机治疗患者。根据随机数表法将其分为对照组与观察组,各 41 例。对照组给予常规护理,观察组在对照组基础上给予基于循证理论的护理。比较两组并发症,入院时和出院时心功能指标、动脉血气指标。结果:观察组并发症发生率为 4。88%,显著低于对照组的 21。95%,差异有统计学意义(P<0。05)。出院时,两组左室射血分数(LVEF)升高,左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)均降低,观察组LVEF高于对照组,LVEDD及LVESD均低于对照组,差异有统计学意义(P<0。05)。出院时,两组动脉血血氧分压(PaO2)、血氧饱和度(SpO2)均升高,动脉血二氧化碳分压(PaCO2)降低,观察组PaO2、SpO2 均高于对照组,PaCO2低于对照组,差异有统计学意义(P<0。05)。结论:重症心力衰竭无创呼吸机治疗患者实施基于循证理论的护理效果较好,可有效预防患者相关并发症发生,改善心功能及动脉血气指标。
Application of Based on Evidence-based Theory Nursing in Non-invasive Ventilator Therapy for Patients with Severe Heart Failure
Objective:To explore the application of based on evidence-based theory nursing in non-invasive ventilator therapy for patients with severe heart failure.Method:A total of 82 patients with severe heart failure treated by non-invasive ventilator in the First Affiliated Hospital of Fujian Medical University from February 2021 to February 2023 were selected.According to random number method,they were divided into control group and observation group,41 cases in each group.The control group was given routine nursing,and the observation group was given based on evidence-based theory nursing on the basis of the control group.Complications,cardiac function index and arterial blood gas index at admission and discharge were compared between the two groups.Result:The incidence of complications in the observation group was 4.88%,which was significantly lower than 21.95%in the control group,and the difference was statistically significant(P<0.05).At discharge,left ventricular ejection fraction(LVEF)increased and left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)decreased in both groups,LVEF in observation group was higher than that in control group,and LVEDD and LVESD were lower than those in control group,and the differences were statistically significant(P<0.05).At discharge,the blood oxygen partial pressure(PaO2)and blood oxygen saturation(SpO2)of both groups were increased,while the partial pressure of carbon dioxide(PaCO2)was decreased,the PaO2 and SpO2 of the observation group were higher than those of the control group,and the PaCO2 was lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion:The effect of based on evidence-based theory nursing is good for patients with severe heart failure treated with non-invasive ventilator,which can effectively prevent the occurrence of related complications and improve the cardiac function and arterial blood gas indexes.
