The Value of ALB,IL-6,NLR in Predicting the Prognosis of Acute Exacerbation of Chronic Obstructive Pulmonary Disease with Respiratory Failure in Elderly Patients
Objective:To investigate the relationship between serum albumin,interleukin-6(IL-6),neutrophil to lymphocyte ratio(NLR)and prognosis in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with respiratory failure(RF).Method:Fifty elderly patients with AECOPD and RF in Wuhan Seventh Hospital from February 2022 to February 2023 were selected as the study subjects.Serum albumin,IL-6 and NLR levels were detected.All patients were treated with mechanical ventilation and atomization inhalation,and the prognosis was evaluated.They were divided into poor prognosis group and good prognosis group.The levels of serum albumin,IL-6 and NLR in the two groups were compared to analyze the correlation between the levels of serum albumin,IL-6 and NLR before treatment and the prognostic value of the elderly patients with AECOPD and RF.Result:In this study,among 50 elderly patients with AECOPD and RF,19 cases had poor prognosis,accounting for 38.00%(19/50),31 cases had good prognosis,accounting for 62.00%(31/50).The serum albumin level in the poor prognosis group before treatment was lower than that in the good prognosis group,and the levels of IL-6 and NLR were higher than those in the good prognosis group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that decreased serum albumin level and increased serum IL-6 and NLR levels were associated with poor prognosis in elderly patients with AECOPD and RF(P<0.05).ROC curve results showed that the AUC of serum albumin,IL-6 and NLR levels before treatment for predicting poor prognosis in elderly AECOPD patients with RF was 0.779,0.849 and 0.750,respectively,which had certain predictive value,the AUC of the three levels combined for predicting poor prognosis in elderly AECOPD patients with RF was 0.917,with high predictive value.Conclusion:The decrease in serum albumin levels and the increase in serum IL-6 and NLR levels are associated with poor prognosis in elderly AECOPD patients with RF.
