Prevention and Treatment Effect of Prophylactic Use of Atropine on Vagal Reflex Induced by Painless Cystoscopy
Objective:To investigate the prevention and treatment effect of prophylactic use of Atropine on vagal reflex induced by painless cystoscopy.Method:From October 2022 to November 2023,144 patients who underwent painless cystoscopy under intravenous combined general anesthesia in Songshan Hospital,Qingdao University Medical College were selected as the research objects,144 selected cases were divided into the control group and the observation group by a completely randomized random number table method, with 72 cases in each group.Dezocine(0.05 mg/kg)combined with 1%Propofol(1.5-2.0 mg/kg)was used for anesthesia induction in both groups, cystoscopy was performed when the eyelash reflex disappeared.Propofol was maintained at 3-6 mg/(kg·h)during the operation,if body movement occurred during the operation,Propofol was added at 0.5 mg/kg until the end of the examination. In the control group,1 mL of 0.9%Sodium Chloride Solution was added to 1%Propofol 20 mL,and 1 mg of Atropine was added to 1%Propofol 20 mL in the observation group. When the heart rate of two groups was less than 50 times/min during anesthesia,0.5 mg of Atropine was injected intravenously. When the blood pressure decreased by more than 20%,6 mg of Ephedrine was injected intravenously. The operation time of painless cystoscopy, postoperative awake time,Propofol dosage, intraoperative vagus reflex and complications were compared between two groups.Result:The operation time of painless cystoscopy in the observation group was shorter than that in the control group, and the difference was statistically significant(P<0.01);there were no significant difference in postoperative awake time and Propofol dosage between two groups(P>0.05).The incidence of vagus reflex in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.01).There were no serious complications in two groups, the control group was cured by timely application of Atropine,Ephedrine and other active rescue treatment. Conclusion:Prophylactic use of Atropine in painless cystoscopy is not only beneficial to shorten the operation time, but also beneficial to respiratory management,it is more conducive to preventing vagus reflex and reducing complications, and has high safety.
