目的:探讨Meek植皮术联合纳米银敷料治疗大面积烧伤患者的效果。方法:回顾性分析2021年7月—2023年7 月南通市第一人民医院收治的 80 例大面积烧伤患者的临床资料,根据治疗方式的不同分为对照组和观察组,各 40 例。对照组给予Meek植皮术治疗,观察组在对照组基础上联合纳米银敷料治疗。对比两组的临床疗效、基质金属蛋白酶(MMPs)水平、炎症因子水平及并发症发生率。结果:观察组创面愈合率、皮片存活率高于对照组,创面愈合时间、皮片融合时间短于对照组,差异有统计学意义(P<0。05)。治疗后,两组金属蛋白酶抑制剂-1(TIMP-1)均高于治疗前,基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-2(MMP-2)均低于治疗前,且观察组TIMP-1 高于对照组,MMP-1、MMP-2 低于对照组,差异有统计学意义(P<0。05)。治疗后,两组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)低于治疗前,白细胞介素-10(IL-10)高于治疗前,且观察组IL-6、TNF-α低于对照组,IL-10 高于对照组,差异有统计学意义(P<0。05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0。05)。结论:Meek植皮术联合纳米银敷料治疗有助于促进大面积烧伤患者的创面愈合,抑制MMPs的表达,减轻炎症反应,减少并发症的发生。
Effect of Meek Skin Grafting Combined with Nano-silver Dressing on Patients with Extensive Burn
Objective:To investigate the effect of Meek skin grafting combined with nano-silver dressing in the treatment of extensive burn patients.Method:The clinical data of 80 patients with extensive burn treated in Nantong First People's Hospital from July 2021 to July 2023 were retrospectively analyzed and divided into control group and observation group according to different treatment methods,with 40 cases in each group.The control group was treated with Meek skin grafting,and the observation group was treated with nano-silver dressing on the basis of control group.The clinical efficacy,matrix metalloproteinases(MMPs)level,inflammatory factor level and complication rate of the two groups were compared.Result:The wound healing rate and skin survival rate of the observation group were higher than those of the control group,and the wound healing time and skin fusion time of the observation group were shorter than those of the control group,the differences were statistically significant(P<0.05).After treatment,tissue inhibitor of metalloproteinases 1(TIMP-1)in both groups were higher than those before treatment,matrix metalloproteinase-1(MMP-1)and matrix metalloproteinase-2(MMP-2)were lower than those before treatment,and TIMP-1 in the observation group was higher than that of the control group,and MMP-1 and MMP-2 were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in two groups were lower than those before treatment,interleukin-10(IL-10)were higher than those before treatment,and IL-6 and TNF-α in observation group were lower than those in control group,and IL-10 in observation group was higher than that in control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Meek skin grafting combined with nano-silver dressing can promote wound healing in patients with extensive burn,inhibit the expression of MMPs,reduce inflammatory response and reduce complications.
