常规吸痰联合定期床旁纤维支气管镜下吸痰治疗重症肺炎机械通气患者的效果评价

目的:探讨常规吸痰联合定期床旁纤维支气管镜下吸痰治疗重症肺炎机械通气患者的效果。方法:选取2020年6 月—2022 年 6 月思南县人民医院收治的 82 例重症肺炎机械通气患者。根据随机数表法将其分为对照组和观察组,各 41例。对照组给予常规吸痰,观察组在对照组基础上给予定期床旁纤维支气管镜下吸痰。比较两组治疗后临床疗效,治疗前及治疗 2 周后血气分析指标、相关指标及恢复情况。结果:观察组总有效率高于对照组,差异有统计学意义(P<0。05)。治疗 2 周后,观察组血氧分压(PaO2)高于对照组,二氧化碳分压(PaCO2)低于对照组,差异有统计学意义(P<0。05)。治疗2 周后,观察组氧浓度(FiO2)、呼气末正压(PEEP)均低于对照组,氧合指数(PaO2/FiO2)高于对照组,差异有统计学意义(P<0。05)。观察组抗菌药物应用时间、肺部炎症吸收时间、机械通气时间及住院时间均短于对照组,差异有统计学意义(P<0。05)。结论:定期床旁纤维支气管镜下吸痰联合常规吸痰可提高重症肺炎机械通气患者疗效,纠正血气分析指标异常,促进肺部炎症吸收,缩短抗菌药物应用及住院时间。
Effect of Regular Sputum Aspiration Combined with Regular Sputum Aspiration under Fiberoptic Tracheoscope at Bed Side in the Treatment of Patients with Severe Pneumonia with Mechanical Ventilation
Objective:To explore the effect of regular sputum aspiration combined with regular sputum aspiration under fiberoptic tracheoscope at bed side in the treatment of patients with severe pneumonia with mechanical ventilation.Method:A total of 82 patients with severe pneumonia treated with mechanical ventilation in Sinan County People's Hospital from June 2020 to June 2022 were selected.They were divided into control group and observation group according to random number table method,with 41 cases in each group.The control group was given regular sputum aspiration,and the observation group was given regular sputum aspiration under fiberoptic bronchoscope at bed side.The clinical efficacy after treatment,blood gas analysis indexes,related indexes before treatment and after 2 weeks of treatment and recovery condition of the two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 2 weeks of treatment,the blood oxygen partial pressure(PaO2)of the observation group was higher than that of the control group,and the carbon dioxide partial pressure(PaCO2)was lower than that of the control group,the differences were statistically significant(P<0.05).After 2 weeks of treatment,the oxygen concentration(FiO2)and positive end expiratory pressure(PEEP)of the observation group were lower than those of the control group,and the oxygenation index(PaO2/FiO2)was higher than that of the control group,the differences were statistically significant(P<0.05).The antibacterial application time,pulmonary inflammation absorption time,mechanical ventilation time and hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Regular sputum aspiration under fiberoptic bronchoscope at the bed side combined with routine sputum aspiration can improve the curative effect of mechanical ventilation in patients with severe pneumonia,correct abnormal blood gas analysis indexes,promote the absorption of lung inflammation,shorten the application of antibiotics and the length of hospital stay.
