Effect of Ivabradine Combined with Different Doses of Sacubitril Valsartan on Patients with Chronic Heart Failure

摘要
目的:分析伊伐布雷定联合不同用量沙库巴曲缬沙坦对慢性心衰患者的影响.方法:选取 2022 年 1-12 月在福州市中医院治疗的 110 例慢性心力衰竭患者,按照随机数表法分为对照组和观察组,各 55 例.两组均服用盐酸伊伐布雷定片,在此基础上,对照组给予沙库巴曲缬沙坦钠片 50 mg/次,观察组给予沙库巴曲缬沙坦钠片 100 mg/次.比较两组的治疗效果、左室射血分数(LVEF)、6 分钟步行试验(6MWT)、左心室舒张末期内径(LVEDD)、血清生长分化因子 15(GDF-15)、细胞间黏附分子-1(ICAM-1)及高敏肌钙蛋白T(hs-TnT)及不良反应发生情况.结果:观察组治疗总有效率、LVEF、6MWT高于对照组,LVEDD、GDF-15、ICAM-1、hs-TnT低于对照组,差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:伊伐布雷定联合高剂量沙库巴曲缬沙坦具有更好的治疗效果,能够进一步改善患者心功能指标与血清指标,提高生命质量.但是在实际使用沙库巴曲缬沙坦时,应根据患者具体情况合理选择剂量,以平衡疗效和安全性.
Abstract
Objective:To analyze the effects of Ivabradine combined with different dosage of Sacubactril Valsartan on patients with chronic heart failure.Method:A total of 110 patients with chronic heart failure treated in Fuzhou Traditional Chinese Medicine Hospital from January to December 2022 were selected and they were divided into control group and observation group according to random number table method,with 55 cases in each group.Both groups were given Ivabradine Hydrochloride Tablets,on the basis of which,the control group was given Sacubactril Valsartan Sodium Tablets 50 mg/time,and the observation group was given Sacubactril Valsartan Sodium Tablets 100 mg/time.The therapeutic effect,left ventricular ejection fraction(LVEF),6 minutes walking test(6MWT),left ventricular end-diastolic diameter(LVEDD),serum growth and differentiation factor 15(GDF-15),intercellular adhesion molecule-1(ICAM-1),high-sensitivity troponin T(hs-TnT)and the incidence of adverse reactions were compared.Result:The total effective rate,LVEF,6MWT in observation group were higher than those in control group,and LVEDD,GDF-15,ICAM-1,hs-TnT were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ivabradine combined with high-dose Sacubitril Valsartan has a better therapeutic effect and can further improve patients'cardiac function and serum indicators,and improve their quality of life.However,when using Sacubitril Valsartan in practice,the dose should be selected reasonably according to the specific conditions of the patients to balance efficacy and safety.
