Application Effect of Health Management Model Based on PDCA in Elderly Diabetic Patients
Objective:To explore the application effect of health management model based on PDCA in elderly diabetic patients.Method:A total of 120 elderly diabetic patients admitted to Foshan Fosun Chancheng Hospital from February 2022 to April 2023 were selected as the main study objects.The control group implemented routine intervention,and the observation group implemented health management model based on PDCA.Blood glucose,quality of life before intervention and after 6 months of intervention,awareness and compliance were compared between the two groups.Result:After 6 months of intervention,fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in both groups were decreased,and fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The cause of the disease,the harm of abnormal blood glucose,the methods of blood glucose control and the points for attention scores in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The scores of scientific diet,regular exercise,blood glucose monitoring and medication as prescribed in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).After 6 months of intervention,quality of life(QOL)scores in both groups were higher than those before intervention,and QOL scores in observation group was higher than that in control group,the differences were statistically significant(P<0.05).Conclusion:Clinical implementation of the health management model based on PDCA for elderly diabetic patients has significant effects in helping patients control blood glucose and improve quality of life,and can also effectively enhance patients'disease cognition degree and improve patients'treatment compliance.
