Nd∶YAP激光联合牙周基础治疗应用于Ⅲ期B/C级牙周炎患者中的临床效果
Clinical Effect of Nd∶YAP Laser Combined with Periodontal Basic Treatment for Stage Ⅲ BC Grade Periodontitis

目的:探讨给予Ⅲ期B/C级牙周炎患者Nd∶YAP激光联合牙周基础治疗的临床效果.方法:选取 2022 年 1 月—2023 年 6 月北京怀柔医院收治的 120 例Ⅲ期B/C级牙周炎患者,以随机数表法分成研究组与对照组,各 60 例.对照组给予牙周基础治疗,研究组给予Nd∶YAP激光联合牙周基础治疗,比较两组临床疗效、牙周状况、龈沟液炎症因子水平、龈下菌群结构指标.结果:研究组治疗总有效率为 98.75%,高于对照组的 87.80%,差异有统计学意义(P<0.05).治疗后 6 周,两组探诊出血位点(BOP)、探诊深度(PD)、临床附着丧失(CAL)、低氧诱导因子-1α(HIF-1α)、细胞间黏附分子-1(ICAM-1)、白细胞介素-6(IL-6)均低于基线时,且研究组低于对照组,差异有统计学意义(P<0.05).治疗后 6 周,两组Shannon指数高于基线时,Simpson指数低于基线时,但研究组Shannon指数低于对照组,Simpson指数高于对照组,差异有统计学意义(P<0.05).结论:Ⅲ期B/C级牙周炎患者给予Nd∶YAP激光联合牙周基础治疗,能够提高临床疗效,改善牙周状况,调节龈沟液炎症因子水平,控制牙周致病菌生长.
Abstract
Objective:To investigate the clinical effect of Nd∶YAP laser combined with periodontal basic treatment in patients with stage Ⅲ B/C grade periodontitis.Method:A total of 120 patients with stage Ⅲ B/C grade periodontitis treated in Beijing Huairou Hospital from January 2022 to June 2023 were selected,and they were divided into study group and control group by random number table method,with 60 cases in each group.The control group was given periodontal basic treatment,and the study group was given Nd∶YAP laser combined with periodontal basic treatment.The clinical efficacy,periodontal status,levels of inflammatory factors in gingival crevicular fluid,and structural indexes of subgingival flora were compared between the two groups.Result:The total effective rate of the study group was 98.75%,which was higher than 87.80%of the control group,and the difference was statistically significant(P<0.05).At 6 weeks after treatment,the bleeding on probing(BOP),probing depth(PD),clinical attachment loss(CAL),hypoxia inducible factor-1α(HIF-1α),intercellular adhesion molecule-1(ICAM-1)and interleukin-6(IL-6)in both groups were lower than those at baseline,and which in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).At 6 weeks after treatment,when Shannon indexes in the two groups were higher than that baseline,Simpson indexes were lower than that baseline,but the Shannon index in the study group was lower than that in the control group,and Simpson index was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Nd∶YAP laser combined with periodontal basic treatment can improve the clinical effect,improve the periodontal condition,regulate the level of inflammatory factors in gingival crevicular fluid and control the growth of periodontal pathogens in patients with stage Ⅲ B/C grade periodontitis.
