目的:探讨血清胱抑素C(cystatin C,Cys C)、N末端B型利钠肽原(N-terminal pro-B-type natriuretic peptide,NT-proBNP)预测慢性肾脏病(chronic kidney disease,CKD)合并心力衰竭(heart failure,HF)的效能.方法:回顾性分析 2020 年 6 月—2022 年 6 月武汉市第七医院收治的 70 例CKD患者的病历资料.根据患者就诊后 1 年内随访资料将其分为发生组(18 例,合并HF)和未发生组(52 例,未合并HF).比较两组基线资料及实验室资料.分析Cys C、NT-proBNP与CKD合并HF的关系.采用受试者工作特征(receiver operating characteristic,ROC)曲线分析Cys C、NT-proBNP对CKD合并HF的预测效能.结果:发生组CKD分期 3 期、4 期占比、Cys C、NT-proBNP水平均显著高于未发生组,差异有统计学意义(P<0.05).经点二列相关性分析显示,Cys C、NT-proBNP与CKD合并HF呈正相关(P<0.05).Cys C、NT-proBNP单独及联合预测CKD合并HF的曲线下面积(area under the curve,AUC)值分别为 0.828、0.868、0.904,联合预测价值最高.结论:血清Cys C、NT-proBNP预测CKD合并HF价值高,两者值越大,CKD合并HF可能性越高.
Abstract
Objective:To investigate the efficacy of serum cystatin C(Cys C)and N-terminal pro-B-type natriuretic peptide(NT-proBNP)in predicting chronic kidney disease(CKD)combined with heart failure(HF).Method:A retrospective study was conducted on the medical records of 70 CKD patients who visited the Seventh Hospital of Wuhan from June 2020 to June 2022.They were divided into occurrence group(18 cases,with HF)and non occurrence group(52 casese,without HF)based on whether HF occurred within one year after the patients'visit.The baseline data and laboratory indexes were compared between the two groups.The relationship between Cys C,NT-proBNP and CKD combined with HF was analyzed.The value of serum Cys C and NT-proBNP in predicting CKD complicated with HF was discussed by receiver operating characteristic(ROC)curve.Result:The proportion of CKD stages 3 and 4,Cys C and NT-proBNP levels in the occurrence group were significantly higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Point two-column correlation analysis showed that Cys C and NT-proBNP were positively correlated with CKD combined with HF(P<0.05).The area under the curve(AUC)values of Cys C and NT-proBNP alone and combined in predicting CKD combined with HF were 0.828,0.868 and 0.904,respectively,and the combined prediction value was the highest.Conclusion:Serum Cys C and NT-proBNP have higher predictive value for CKD combined with HF,and the higher the values,the higher the likelihood of CKD combined with HF.
