持续氧气驱动雾化吸入治疗对老年卧床患者的影响

目的:分析持续氧气驱动雾化吸入治疗对老年卧床患者的影响。方法:选取 2021 年 1 月—2023 年 6 月广州广钢新城医院收治的 120 例老年卧床患者。随机将其均分为对照组和观察组,各 60 例。对照组使用超声雾化吸入,观察组使用持续氧气驱动雾化吸入。比较两组治疗前后肺功能、血气指标、呼吸功能,排痰量及坠积性肺炎发生率。结果:治疗后,观察组第 1 秒用力呼气容积(FEV1)、用力肺活量(FVC)均高于对照组,差异有统计学意义(P<0。05)。治疗后,观察组动脉血二氧化碳分压(PaCO2)低于对照组,动脉血氧分压(PaO2)、血氧饱和度(SaO2)均高于对照组,差异有统计学意义(P<0。05)。治疗后,两组氧合指数(PaO2/FiO2)高于治疗前,吸入氧浓度(FiO2)低于治疗前,观察组PaO2/FiO2 高于对照组,FiO2 低于对照组,差异有统计学意义(P<0。05)。观察组排痰量情况优于对照组,坠积性肺炎发生率低于对照组,差异有统计学意义(P<0。05)。结论:老年卧床患者采用持续氧气驱动雾化吸入治疗可有效预防坠积性肺炎发生,患者的肺功能水平明显提升,其血气指标趋于正常水平。
Effect of Continuous Oxygen Driven Atomization Inhalation Therapy on Elderly Bedridden Patients
Objective:To analyze the effect of continuous oxygen driven atomization inhalation therapy on elderly bedridden patients.Method:A total of 120 elderly bedridden patients treated in Guangzhou Steel New City Hospital from January 2021 to June 2023 were selected.They were randomly divided into control group and observation group,60 cases in each group.The control group received ultrasonic atomization inhalation,and the observation group received atomization inhalation driven by continuous oxygen.Lung function,blood gas index,respiratory function before and after treatment,sputum output and incidence of hypostatic pneumonia were compared between the two groups.Result:After treatment,forced expiratory volume in the first second(FEV1)and forced vital capacity(FVC)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the arterial partial pressure of carbon dioxide(PaCO2)in the observation group was lower than that in the control group,and the arterial partial pressure of oxygen(PaO2)and the blood oxygen saturation(SaO2)were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the oxygenation index(PaO2/FiO2)of the two groups was higher than that before treatment,and inhaled oxygen concentration(FiO2)was lower than that before treatment,the PaO2/FiO2 of the observation group was higher than that of the control group,and FiO2 was lower than that of the control group,the differences were statistically significant(P<0.05).The sputum output of the observation group was better than that of the control group,and the incidence of hypostatic pneumonia was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion:In elderly bedridden patients,continuous oxygen-driven atomization inhalation can effectively prevent the occurrence of hypostatic pneumonia,and the lung function level of patients is significantly improved,and the blood gas index tends to normal level.
