目的:分析小儿豉翘清热颗粒对儿童支原体肺炎患儿肺功能、炎症因子及辅助性T细胞 17(Th17)/调节性T细胞(Treg)的影响.方法:选择 2021 年 10 月—2023 年 4 月在麻城市第二人民医院儿科诊疗的 100 例儿童支原体肺炎患儿作为研究对象,按照治疗方案不同将患儿分为对照组(n=48)和治疗组(n=52).对照组进行常规治疗,治疗组在常规治疗基础上使用小儿豉翘清热颗粒进行治疗.比较两组肺功能[第 1 秒用力呼气容积(FEV1)、用力肺活量(FVC)、呼气峰值流速(PEF)]、炎症因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、CD4+T淋巴细胞亚群(Th17、Treg、Th17/Treg)、相关通路蛋白表达[磷脂酰肌醇 3 激酶(PI3K)、蛋白激酶B(Akt)、信号转导及转录激活蛋白 3(STAT3)].结果:治疗后,两组FEV1、FVC、PEF、Treg高于治疗前,CRP、TNF-α、IL-6、Th17、Th17/Treg水平及PI3K、Akt、STAT3 mRNA相对表达量低于治疗前,且治疗组FEV1、FVC、PEF、Treg高于对照组,CRP、TNF-α、IL-6、Th17、Th17/Treg水平及PI3K、Akt、STAT3 mRNA相对表达量低于对照组,差异有统计学意义(P<0.05).结论:小儿豉翘清热颗粒可以通过抑制PI3K、Akt、STAT3 信号通路的激活,调控Th17/Treg细胞因子平衡,从而发挥治疗儿童支原体肺炎的效果,还可以改善肺功能,降低炎症因子水平.
Effect of Xiao'er Chiqiao Qingre Granules on Lung Function,Inflammatory Factors and Th17/Treg in Children with Mycoplasma Pneumonia
Objective:To analyze the effect of Xiao'er Chiqiao Qingre Granules on lung function,inflammatory factors and T helper cell 17(Th17)/regulatory T cell(Treg)in children with mycoplasma pneumonia.Method:A total of 100 children with mycoplasma pneumonia who diagnosed and treated in the Department of Pediatrics of Macheng Second People's Hospital from October 2021 to April 2023 were selected as the research objects,the children were divided into the control group(n=48)and treatment group(n=52)according to different treatment schemes.The control group was treated with routine treatment,and the treatment group was treated with Xiao'er Chiqiao Qingre Granules on the basis of routine treatment.The lung function[forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),peak expiratory flow(PEF)],inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],CD4+T lymphocyte subsets(Th17,Treg,Th17/Treg),and related pathway protein expression[phosphatidylinositol 3-kinase(PI3K),protein kinase B(Akt),signal transducer and activator of transcription 3(STAT3)]were compared between the two groups.Result:After treatment,the FEV1,FVC,PEF and Treg in two groups were higher than those before treatment,and the levels of CRP,TNF-α,IL-6,Th17,Th17/Treg and the relative expression of PI3K,Akt and STAT3 mRNA were lower than those before treatment,the FEV1,FVC,PEF and Treg in the treatment group were higher than those in the control group,the levels of CRP,TNF-α,IL-6,Th17,Th17/Treg and the relative expression of PI3K,Akt and STAT3 mRNA in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Xiao'er Chiqiao Qingre Granules can regulate the balance of Th17/Treg cytokines by inhibiting the activation of PI3K,Akt and STAT3 signaling pathways,so as to exert the effect of treating children with mycoplasma pneumonia,it can also improve lung function and reduce the level of inflammatory factors.
