温中健脾方联合肠内营养粉剂(TP)在老年髋关节置换术患者围手术期中的应用效果

目的:探讨温中健脾方联合肠内营养粉剂(TP)在老年髋关节置换术患者围手术期中的应用效果。方法:选取2021 年 1 月—2023 年 8 月厦门市中医院骨科收治的 100 例老年髋关节置换术患者。在骨二科病区随机选取 50 例患者纳入试验组,在骨三科病区随机选取 50 例患者纳入对照组。试验组围手术期使用温中健脾方联合肠内营养粉剂(TP)进行胃肠内营养,对照组常规术前禁饮禁食、术后间断禁食禁水。比较两组的机体免疫相关指标、营养相关指标、康复进程指标(肛门排气时间、经口进食时间、下床时间)及并发症发生率等。结果:术前,两组免疫相关指标、营养相关指标比较,差异无统计学意义(P>0。05);术后 3 d,试验组IgA、IgM、IgG、前白蛋白(PA)、白蛋白(ALB)、血红蛋白(Hb)均高于对照组,差异有统计学意义(P<0。05)。试验组肛门排气时间、经口进食时间、下床时间均早于对照组,差异有统计学意义(P<0。05)。试验组并发症发生率为 2。00%,低于对照组的 10。00%,差异有统计学意义(P<0。05)。结论:老年髋关节置换术患者围手术期予以温中健脾方联合肠内营养粉剂(TP)进行胃肠内营养,可帮助改善患者机体营养,提高机体对手术的应激反应能力,提高免疫,促进术后康复,减少并发症发生。
Clinical Effect of Wenzhong Jianpi Formula Combined with Enteral Nutritional Powder(TP)in the Perioperative Period of Elderly Patients with Hip Replacement
Objective:To investigate the effect of Wenzhong Jianpi Formula combined with Enteral Nutritional Powder(TP)in the perioperative period of elderly patients with hip replacement.Method:A total of 100 elderly patients with hip replacement admitted to the Department of Orthopedics of Xiamen Traditional Chinese Medicine Hospital from January 2021 to August 2023 were selected.Fifty patients were randomly selected from the Second Department of Orthopedics to be included in the experimental group,and 50 patients from the Third Department of Orthopedics to be included in the control group.The experimental group was treated with Wenzhong Jianpi Formula combined with Enteral Nutrition Powder(TP)for gastrointestinal nutrition during perioperative period,while the control group was prohibited from drinking and fasting before operation and intermittent fasting and water abstinence after operation.The immune-related indexes,nutrition-related indexes,recovery process indexes(anal exhaust time,oral feeding time,getting out of bed time)and complication rate were compared between the two groups.Result:Before surgery,there were no significant differences in immune-related indexes and nutrition-related indexes between the two groups(P>0.05).At 3 d after surgery,IgA,IgM,IgG,prealbumin(PA),albumin(ALB),hemoglobin(Hb)in experimental group were higher than those in control group,the differences were statistically significant(P<0.05).The anal exhaust time,oral feeding time and getting out of bed time of experimental groups were earlier than those of control group,and the differences were statistically significant(P<0.05).The incidence of complications in experimental group was 2.00%,which was lower than 10.00%in control group,and the difference was statistically significant(P<0.05).Conclusion:During perioperative period,Wenzhong Jianpi Formula combined with Enteral Nutrition Powder(TP)can improve the body nutrition,improve the body's ability to respond to stress during surgery,improve immunity,promote postoperative rehabilitation and reduce complications in elderly patients with hip replacement.
