目的:分析高压氧联合介入栓塞术在颅内动脉瘤患者中的应用价值及对神经功能、炎症反应的影响。方法:选取 2022 年 1 月—2023 年 9 月松滋市人民医院神经外科收治住院的 80 例颅内动脉瘤患者作为研究对象,随机分为对照组与研究组,各 40 例。对照组采用介入栓塞术治疗,研究组采用高压氧联合介入栓塞术治疗。比较两组大脑中动脉情况、基质金属蛋白酶 9(matrix metalloproteinase 9,MMP-9)、胱抑素C(cystatin C,Cys C)、炎症因子[白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-10(interleukin-10,IL-10)]、神经功能[美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)]、日常生活能力[Barthel指数(Barthel index,BI)]、并发症及临床疗效。结果:与对照组相比,研究组治疗后大脑中动脉血流速度、脑血管痉挛(cerebro vascular spasm,CVS)情况、MMP-9、Cys C、IL-6、TNF-α水平及NIHSS评分较低,BI评分及IL-10 水平较高,差异有统计学意义(P<0。05);与对照组相比,研究组并发症发生率较低,治疗总有效率较高,差异有统计学意义(P<0。05)。结论:高压氧与介入栓塞术联用,对颅内动脉瘤患者神经功能有明显改善作用,抑制炎症因子水平,减少并发症,提高临床疗效,促进患者恢复。
Application Value of Hyperbaric Oxygen Combined with Interventional Embolization in Patients with Intracranial Aneurysm and Its Effect on Nerve Function and Inflammatory Response
Objective:To analyze the application value of hyperbaric oxygen combined with interventional embolization in patients with intracranial aneurysm and its effect on nerve function and inflammatory response.Method:A total of 80 patients with intracranial aneurysm who admitted to the Neurosurgery Department of Songzi City People's Hospital from January 2022 to September 2023 were selected as the study objects,they were randomly divided into the control group and the study group,40 cases in each group.The control group was treated with interventional embolization,and the study group was treated with hyperbaric oxygen combined with interventional embolization.The middle cerebral artery,matrix metalloproteinase 9(MMP-9),cystatin C(Cys C),inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)],neurological function[National Institute of Health stroke scale(NIHSS)],daily living ability[Barthel index(BI)],complications and clinical efficacy were compared between two groups.Result:Compared with the control group,the middle cerebral artery flow rate,cerebro vascular spasm(CVS),MMP-9,Cys C,IL-6,TNF-α levels and NIHSS scores in the study group after treatment were lower,and the BI score and IL-10 level were higher,the differences were statistically significant(P<0.05).Compared with the control group,the incidence of complications in the study group was lower,and the total treatment effective rate of treatment was higher,the differences were statistically significant(P<0.05).Conclusion:The combination of hyperbaric oxygen and interventional embolization can significantly improve the nerve function of patients with intracranial aneurysm,inhibit the level of inflammatory factors,reduce complications,improve clinical efficacy,and promote patients recovery.
