Value of Serum KIM-1,SFRP5 and CHI3L1 Combined in the Diagnosis of Diabetes Nephropathy
Objective:To investigate the value of serum kidney injury molecule-1(KIM-1),secreted frizzled-related protein 5(SFRP5),chitinase 3-like protein 1(CHI3L1)combined in the diagnosis of diabetes nephropathy(DN).Method:A total of 120 patients with DN treated in the Outpatient Department of Nanping First Hospital from May 2020 to May 2023 were selected as DN group,and 120 patients with simple T2DM treated in the Outpatient Department during the same period were selected as the control group.The basic data of two groups were collected,the levels of serum KIM-1,SFRP5 and CHI3L1 in the two groups were detected.The DN group was divided into early group and middle/late group according to early DN diagnostic criteria.The general data and levels of serum KIM-1,SFRP5 and CHI3L1 in DN group and control group were compared.The levels of serum KIM-1,SFRP5 and CHI3L1 in early group and middle/late group were compared.The influencing factors of DN was analyzed.The diagnostic and practical value of serum KIM-1,SFRP5 and CHI3L1 for DN were analyzed.Result:The course of T2DM in DN group was longer than that in control group,and glycosylated hemoglobin(HbA1c),cystatin C(Cys C)were significantly higher than those in control group,the differences were statistically significant(P<0.05).The levels of serum KIM-1 and CHI3L1 in DN group were significantly higher than those in control group,and the level of SFRP5 was significantly lower than that in control group,the differences were statistically significant(P<0.05).The levels of serum KIM-1 and CHI3L1 in middle/late group were significantly higher than those in early group,and the level of SFRP5 was significantly lower than that in early group,the differences were statistically significant(P<0.05).The results showed that the increase of serum KIM-1 and CHI3L1 was an independent risk factor for the development of DN,and the increase of SFRP5 was a protective factor for the development of DN(P<0.05).The truncation values of serum KIM-1,SFRP5 and CHI3L1 were 5.38 ng/mL,4.21 μg/L and 82.25 pg/mL,and the areas under the curve were 0.752,0.817 and 0.756 respectively.The area under the combined curve was 0.896,which was higher than that under the individual curve of each index,and the difference was statistically significant(Z=3.788,2.268,3.683,P<0.05).When the horizontal high-risk threshold was between 0.02 and 0.97,the net benefit rate of the combined diagnosis of DN by serum KIM-1,SFRP5 and CHI3L1 was higher than that by each index alone.Conclusion:The levels of serum KIM-1 and CHI3L1 in DN patients increase,while the level of SFRP5 decreases,the combined detection of three has high clinical value for the diagnosis of DN.
