Clinical Effect of Transurethral Holmium Laser Enucleation of Theprostate in the Treatment of Elderly Prostatic Hyperplasia and Its Influence on Sexual Function
Objective:To investigate the clinical effect of transurethral holmium laser enucleation of theprostate(HoLEP)and transurethral plasma kinetic enucleation of prostate(PKEP)in the treatment of senility prostatic hyperplasia(PH)and its influence on sexual function.Method:A total of 60 elderly PH patients admitted to Huai'an Huai'an Hospital from January 2022 to February 2023 were selected as the study objects,and they were randomly divided into study group(receiving HoLEP treatment)and control group(receiving PKEP treatment),with 30 cases in each group.Perioperative indexes,urinary flow dynamics indexes,sexual function indexes and postoperative complications were compared between the two groups.Result:The operation time,bladder irrigation time,catheter retention time and hospital stay of the study group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the prostate tissue resection amount was higher than that of the control group,the differences were statistically significant(P<0.05).Three months after operation,the maximum urine flow rate and average urine flow rate of the study group were higher than those of the control group,and the residual urine volume of the bladder was less than that of the control group,the differences were statistically significant(P<0.05).Three months after operation,the international index of erectile function-5(IIEF-5)score of the study group was higher than that of the control group,and the incidence of retrograde ejaculation was lower than that of the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the study group was 13.33%,which was lower than 33.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion:HoLEP treatment for elderly PH patients has shorter operation time,less intraoperative bleeding and faster recovery after surgery,which can significantly improve postoperative urodynamics and sexual function,with fewer complications and high safety.
