中文摘要：目的:探讨通腑解毒汤联合常规西医治疗重症急性胰腺炎(SAP)的临床效果。方法:选取2021年1月—2023年12 月毕节市中医医院收治的 65 例SAP患者作为研究对象，按随机数表法分为对照组(n=33)及观察组(n=32)。对照组行常规西医治疗，观察组加用通腑解毒汤治疗，持续 10 d。比较两组临床疗效、症状消失时间、血流动力学指标、炎症水平、急性生理与慢性健康状况(APACHEⅡ)评分、Marshall多脏器功能障碍(MODS)评分。结果:观察组总有效率高于对照组，差异有统计学意义(P<0。05)。观察组发热消失时间、腹痛消失时间、呕吐消失时间、血清淀粉酶复常时间及首次排便时间短于对照组，差异有统计学意义(P<0。05)。治疗后，观察组心率(HR)、血乳酸(BL)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)低于对照组，平均动脉压(MAP)高于对照组，差异有统计学意义(P<0。05)。治疗后，观察组APACHEⅡ评分、MODS评分低于对照组，差异有统计学意义(P<0。05)。结论:通腑解毒汤联合常规西医治疗SAP患者效果显著，可加快体内炎症消退，纠正心率、血压异常，促进疾病症状消失。

外文摘要：Objective:To explore the clinical effect of Tongfu Jiedu Decoction combined with conventional western medicine on severe acute pancreatitis(SAP).Method:A total of 65 SAP patients admitted to Bijie Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were selected as the study objects,and they were divided into control group(n=33)and observation group(n=32)according to random number table method.The control group received conventional western medicine treatment,and the observation group was treated with Tongfu Jiedu Decoction for 10 d.The clinical efficacy,symptom resolution time,hemodynamic indexes,inflammation level,acute physiology and chronic health evaluation(APACHEⅡ)score and Marshall multiple organ dysfunction syndrome(MODS)score were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The disappearance time of fever,abdominal pain,vomiting,serum amylase normalization time and first defecation time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the heart rate(HR),blood lactic acid(BL),interleukin-6(IL-6),interleukin-8(IL-8),hypersensitive C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)in observation group were lower than those in control group,and the mean arterial pressure(MAP)was higher than that in control group,the differences were statistically significant(P<0.05).After treatment,the APACHEⅡ score and MODS score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Tongfu Jiedu Decoction combined with conventional western medicine is effective in the treatment of SAP patients,which can accelerate the regression of inflammation in the body,correct abnormal heart rate and blood pressure,and promote the disappearance of disease symptoms.
