基于超声心动图的不同评估方法在川崎病患儿冠状动脉扩张中的诊断效能

目的:探讨基于超声心动图(ECHO)的不同评估方法在川崎病(KD)患儿冠状动脉扩张中的诊断效能。方法:回顾性分析 2021 年 6 月—2023 年 5 月榆林市榆阳区人民医院收治的 80 例KD患儿的病例资料。所有患者进行ECHO检查,并进行冠状动脉造影检查判定,1995 标准,郑淋Z值法,Kobayashi法与Dallaire法评估。分析所有患儿冠状动脉扩张情况。分析ECHO下不同评估方法诊断结果及其与金标准(冠状动脉造影)的一致性。比较不同评估方法诊断KD患儿冠状动脉扩张的效能。结果:所有患儿随访 6 个月后,冠状动脉造影检查判定标准显示,冠状动脉扩张 43 例(53。75%),未出现冠状动脉扩张 37 例(46。25%)。1995 标准诊断KD患儿冠状动脉扩张的敏感度为 72。09%(31/43),特异度为 81。08%(30/37),准确度为 76。25%(61/80),Kappa值为 0。527。郑淋Z值法诊断KD患儿冠状动脉扩张的敏感度为 69。77%(30/43),特异度为 86。49%(32/37),准确度为 77。50%(62/80),Kappa值为 0。554。Kobayashi法诊断KD患儿冠状动脉扩张的敏感度为 88。37%(38/43),特异度为 94。59%(35/37),准确度为 91。25%(73/80),Kappa值为 0。825。Dallaire法诊断KD患儿冠状动脉扩张的敏感度为 76。74%(33/43),特异度为 83。78%(31/37),准确度为 80。00%(64/80),Kappa值为 0。601。Kobayashi法诊断KD患儿冠状动脉扩张的敏感度高于郑淋Z值法,准确度高于 1995 标准、郑淋Z值法、Dallaire法,差异有统计学意义(P<0。05)。结论:在几种评估法中,Kobayashi法对于KD患儿冠状动脉扩张诊断准确度具有较大优势。
Diagnostic Efficacy of Different Evaluation Methods Based on Echocardiography for Coronary Artery Dilatation in Children with Kawasaki Disease
Objective:To explore the diagnostic efficacy of different evaluation methods based on echocardiogram(ECHO)in artery dilatation of children with Kawasaki disease(KD).Method:The case data of 80 children with KD treated in Yulin People's Hospital of Yuyang District from June 2021 to May 2023 were retrospectively analyzed.ECHO examination was performed in all patients,and all patients underwent coronary angiography examination determine,1995 standard,Zheng Lin Z-value method,Kobayashi method and Dallaire method evaluation.Coronary artery dilatation condition was analyzed in all children.The diagnostic results of different evaluation methods based on ECHO and their consistency with gold standard(coronary angiography)were analyzed.The efficacy of different evaluation methods in diagnosing coronary artery dilatation in children with KD was compared.Result:After 6 months of follow-up,the criteria of coronary angiography examination showed that 43 cases(53.75%)had coronary artery dilation and 37 cases(46.25%)had no coronary artery dilation.The sensitivity,specificity and accuracy for the 1995 standard diagnosis of coronary artery dilation in KD children were 72.09%(31/43),81.08%(30/37),and 76.25%(61/80)respectively,the Kappa value was 0.527.The sensitivity,specificity,accuracy of Zheng Lin Z value method for diagnosing coronary artery dilation in children with KD were 69.77%(30/43),86.49%(32/37),77.50%(62/80)and Kappa value was 0.554 respectively.The sensitivity and specificity,accuracy of Kobayashi method for diagnosing coronary artery dilation in KD children were 88.37%(38/43),94.59%(35/37),91.25%(73/80)respectively and Kappa value was 0.825.The sensitivity,specificity,and accuracy of Dallaire method for diagnosing coronary artery dilation in KD children were 76.74%(33/43),83.78%(31/37),and 80.00%(64/80)respectively,the Kappa value was 0.601.The sensitivity of Kobayashi method in diagnosing coronary artery dilatation in KD children was higher than that of Zheng Lin Z-value method,the accuracy was higher than that in 1995 standard,Zheng Lin Z-value method and Dallaire method,and the differences were statistically significant(P<0.05).Conclusion:Among several evaluation methods,Kobayashi method has a greater advantage in the diagnostic accuracy of coronary artery dilation in children with KD.
