MRI for Differential Diagnosis of Phyllode Tumor of the Breast and Breast Fibroadenoma
Objective:To investigate the differential diagnostic value of phyllode tumor of the breast(PTB)and breast fibroadenoma.Method:A total of 45 patients with PTB and 45 patients with breast fibroadenoma treated in Liupanshui People's Hospital from May 2021 to May 2023 were selected as the study objects.All patients underwent MRI examination,and statistical MRI detection of PTB and breast fibroadenoma was performed,and the surgical results were used as the gold standard to analyze the efficacy of MRI in the differential diagnosis of PTB and breast fibroadenoma.Kappa test was used to verify the consistency of MRI diagnosis and surgical results.The differences between PTB and breast fibroadenoma were compared in tumor shape,tumor size,enhanced boundary shape,cystic degeneration,low signal separation,type of time-signal intensity curve(TIC),peri-tumor halo sign,T2WI fat suppression signal,apparent diffusion coefficient(ADC)value,and T1WI high signal.Result:Among the 90 patients,a total of 44 cases of PTB and 46 cases of breast fibroadenoma were detected by MRI.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of MRI were 97.78%,100.00%,100.00%,and 97.83%respectively in the differential diagnosis of PTB and breast fibroadenoma.Kappa test showed that MRI was in good agreement with gold standard(Kappa=0.978,P<0.001).There were significant differences in tumor morphology,tumor size,cystic change,peritumoral halo sign,fat suppression signal on T2WI and high signal on T1WI between the two groups(P<0.05).Conclusion:MRI has a high value in the differential diagnosis of PTB and breast fibroadenoma,which can improve the diagnostic accuracy and facilitate the formulation of subsequent targeted treatment plans.
