Application Effect of Lower Body Inflatable Insulation in Elderly Patients Undergoing Off-pump Coronary Artery Bypass Grafting
Objective:To explore the application effect of lower body inflatable insulation in elderly patients undergoing off-pump coronary artery bypass grafting.Method:A total of 232 elderly patients who undergoing off-pump coronary artery bypass grafting in Fujian Cardiology Center-operating Room from January 1,2022 to December 31,2023 were retrospectively selected as the research objects,they were divided into the control group(108 cases)and the observation group(124 cases)according to different intraoperative heat preservation measures.The control group was used inflatable thermal insulation blanket to cover the upper body on the basis of simple thermal insulation measures in the conventional operating room,and the observation group was used inflatable thermal insulation blanket to cover the lower body on the basis of simple thermal insulation measures in the conventional operating room.The perioperative core body temperature,postoperative peripheral circulation recovery,adverse reactions within 24 h after operation and inflammatory factor levels were compared between two groups.Result:Compared with the control group,the core body temperatures of the observation group at the disinfection of surgical field,after thoracotomy,1 h after thoracotomy,before anastomosis of blood vessels,after anastomosis of blood vessels and the end of surgery were higher,and the differences were statistically significant(P<0.05);compared with the control group,the proportion of ruddy skin color and warm skin temperature in the observation group were higher,and the capillary filling time was shorter,the differences were statistically significant(P<0.05);compared with the control group,the total incidence of chills and adverse reactions in the observation group within 24 h after operation were lower,and the differences were statistically significant(P<0.05);the levels of tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),interleukin-2(IL-2),interleukin-4(IL-4)and interleukin-6(IL-6)in the observation group at 5 min and 24 h after operation were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Lower body inflatable insulation is helpful to maintain the stability of perioperative core body temperature in elderly patients with off-pump coronary artery bypass grafting,accelerate the recovery of postoperative peripheral circulation,reduce the incidence of postoperative chills,and has less influence on the immune balance of body.
