超声检测脐动脉血流参数对孕晚期胎儿生长受限的诊断价值分析

目的:探讨超声检测脐动脉血流参数对孕晚期胎儿生长受限(FGR)的诊断价值。方法:选择 2021 年 6 月—2023 年 6 月于莆田市儿童医院进行产检的 634 例孕妇作为研究对象,所有入选孕妇均行超声检查,均由同一位经验丰富的超声医师完成检查。以临床诊断结果将 634 例孕妇分为FGR组与发育正常组,比较两组各项脐动脉血流动力学参数水平,包括阻力指数(RI)、搏动指数(PI)、脐动脉收缩末期峰值/舒张末期峰值(S/D)、最小血流速度(Min)、平均时间血流速度(TAV)、最大血流速度(Max)。另外,以临床诊断结果作为参考,分析超声检查诊断FGR的临床价值。结果:634例孕妇经临床证实,38 例存在FGR,占 5。99%(38/634),596 例孕妇检查胎儿发育正常,占 94。01%(596/634)。FGR组脐动脉Min(0。13±0。05)、Max(0。42±0。08)、TAV(0。25±0。06)均低于发育正常组(0。21±0。06)、(0。53±0。12)、(0。34±0。08),PI(1。12±0。21)、RI(0。67±0。11)、S/D(3。11±0。73)高于发育正常组(0。91±0。14)、(0。54±0。07)、(2。42±0。45),差异有统计学意义(P<0。05)。超声检查诊断FGR的敏感度为 94。74%(36/38),特异度为 98。99%(590/596),准确度为 98。74%(626/634),阳性预测值为 85。71%(36/42),阴性预测值为 99。66%(590/592)。结论:超声检测脐动脉血流参数对诊断FGR有较高的参考价值,诊断准确率较高,可提升诊断效能。
Diagnostic Value of Ultrasound Detection of Umbilical Artery Blood Flow Parameters for Fetal Growth Restriction in Late Pregnancy
Objective:To explore the diagnostic value of ultrasound detection of umbilical artery blood flow parameters for fetal growth restriction(FGR)in late pregnancy.Method:A total of 634 pregnant women who underwent prenatal examination in Putian Children's Hospital from June 2021 to June 2023 were selected as the study subjects,all selected pregnant women were underwent ultrasound examination,all of which were completed by the same experienced ultrasound physician.634 pregnant women were divided into FGR group and normal development group based on clinical diagnostic results,the levels of various umbilical artery hemodynamic parameters,including resistance index(RI),pulsatility index(PI),peak systolic/diastolic(S/D),minimum blood flow velocity(Min),mean time blood flow velocity(TAV),and maximum blood flow velocity(Max)were compared between two groups.In addition,using clinical diagnostic results as a reference,the clinical value of ultrasound examination in diagnosing FGR was analyzed.Result:Among the 634 cases of pregnant women confirmed by clinical,there were 38 cases of FGR,accounting for 5.99%(38/634),596 cases of pregnant women check normal fetal development,accounting for 94.01%(596/634).The Min(0.13±0.05),Max(0.42±0.08)and TAV(0.25±0.06)of umbilical artery in FGR group were lower than(0.21±0.06),(0.53±0.12)and(0.34±0.08)in normal development group,PI(1.12±0.21),RI(0.67±0.11)and S/D(3.11±0.73)were higher than(0.91±0.14),(0.54±0.07)and(2.42±0.45)in normal development group,the differences were statistically significant(P<0.05).The sensitivity of ultrasound examination for diagnosing FGR was 94.74%(36/38),specificity was 98.99%(590/596),accuracy was 98.74%(626/634),positive predictive value was 85.71%(36/42),and negative predictive value was 99.66%(590/592).Conclusion:Ultrasound detection of umbilical artery blood flow parameters has high reference value for diagnosing FGR,with high diagnostic accuracy and can improve diagnostic efficiency.
