Effect of LC Combined with ESBD in the Treatment of Cholecystolithiasis Complicated with Choledocholithiasis
Objective:To investigate the effect of laparoscopic cholecystectomy(LC)combined with endoscopic sphincterotomy with balloon dilatation(ESBD)in the treatment of cholecystolithiasis complicated with choledocholithiasis.Method:Ninety-five patients with cholecystolithiasis combined with choledocholithiasis admitted to the Fifth People's Hospital of Huai'an from May 2022 to June 2023 were retrospectively selected.They were divided into control group(47 cases)and observation group(48 cases)according to different operation methods.The control group received LC combined with laparoscopic transcystic common bile duct exploration(LTCBDE),and the observation group received LC combined with ESBD.Perioperative indexes,preoperative and postoperative serum biochemical indexes,inflammatory factors and complications were compared between the two groups.Result:The operation time of observation group was shorter than that of control group,and the first postoperative exhaust time was earlier than that of control group,the differences were statistically significant(P<0.05).At 3 d after surgery,the levels of direct bilirubin(DBIL),alkaline phosphatase(ALP)and γ-glutamyltransferase(GGT)in the two groups were lower than those before surgery,and the levels of DBIL,ALP and GGT in observation group were lower than those in control group,the differences were statistically significant(P<0.05).At 3 d after surgery,high mobility group protein B1(HMGB1)and procalcitonin(PCT)in the two groups were higher than those before surgery,carbohydrate antigen 19-9(CA19-9)was lower than that before surgery,HMGB1,PCT and CA19-9 in observation group were lower than control group,the differences were statistically significant(P<0.05).There was no significant difference in complications between the two groups(P>0.05).Conclusion:Both LC combined with LTCBDE and LC combined with ESBD can effectively remove stones,but LC combined with ESBD can further shorten the operative time,promote the recovery of postoperative liver function,and reduce postoperative inflammation reaction.
