Impact of Collaborative Care Based on Cross-theoretical Model on Peritoneal Dialysis Patients
Objective:To explore the impact of collaborative care based on cross-theoretical model on peritoneal dialysis patients.Method:A total of 120 patients who underwent peritoneal dialysis in Wuxi People's Hospital Affiliated to Nanjing Medical University from March 2021 to March 2023 were included in the study,grouped by touching the ball,the odd number was the control group,and the even number was the observation group,with 60 cases in each group.The control group was given routine nursing,and the observation group was given collaborative care based on cross-theoretical model.The self-care ability[self-care ability scale(ESCA)],nutritional status[hemoglobin(Hb),albumin(ALB)and prealbumin(PAB)],negative emotions[Beck anxiety inventory(BAI),Beck depression inventory(BDI),Connor-Davidson resilience scale(CD-RISC)]and quality of life[kidney disease quality of life scale(KDQOL)]were compared between two groups.Result:The scores of self-concept,self-responsibility,self-care skills,health knowledge level,Hb,ALB,PAB,CD-RISC score,scores of mental health,physical health,kidney burden,influence of kidney disease,symptoms and discomfort in the observation group were higher than those in the control group,while the BAI score and BDI score were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Collaborative care based on cross-theoretical model can improve the self-care ability of peritoneal dialysis patients,improve nutritional status,reduce negative emotions,and improve the quality of life.
