目的:探讨神经肌肉电刺激(neuromuscular electrical stimulation,NMES)联合循证护理在ICU获得性衰弱(ICU-acquired weakness,ICU-AW)患者中的应用价值。方法:选取2021年8月—2023年8月南平市第一医院收治的98例ICU-AW患者。根据随机数表法将其分为观察组和对照组,各49例。对照组给予常规护理,观察组给予NMES联合循证护理。比较两组干预后肌力、日常生活能力、病情严重程度、睡眠情况,时间指标及不良事件。结果:干预后,观察组ICU-AW发生率和急性生理与慢性健康状况量表Ⅱ(acute physiology and chronic health status scale Ⅱ,APACHE Ⅱ)评分均低于对照组,医学研究委员会(Medical Research Council,MRC)评分和Barthel评分均高于对照组,差异有统计学意义(P<0。05)。观察组机械通气时间和ICU治疗时间均短于对照组,匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)评分低于对照组,差异有统计学意义(P<0。05)。两组均无严重不良事件发生。结论:NMES联合循证护理可以改善ICU-AW患者的肌力和功能,降低ICU-AW发生率、缩短机械通气时间和ICU治疗时间,并改善睡眠质量,且未发生严重不良事件。
Application Value of NMES Combined with Evidence-based Nursing in ICU-AW Patients
Objective:To explore the application value of neuromuscular electrical stimulation (NMES) combined with evidence-based nursing in ICU-acquired weakness (ICU-AW) patients.Method:A total of 98 ICU-AW patients admitted to Nanping First Hospital City from August 2021 to August 2023 were selected.They were divided into observation group and control group according to random number table method,49 cases in each group.The control group was given routine nursing,and the observation group was given NMES combined evidence-based nursing.Muscle strength,daily living ability,severity of disease,sleep status after intervention,time index and adverse events were compared between the two groups.Result:After intervention,the incidence of ICU-AW and acute physiological and chronic health status scale Ⅱ (APACHE Ⅱ) score in observation group were lower than those in control group,and Medical Research Council (MRC) score and Barthel score were higher than those in control group,the differences were statistically significant (P<0.05).The mechanical ventilation time and ICU treatment time in the observation group were shorter than those in the control group,and the Pittsburgh sleep quality index (PSQI) score was lower than that in the control group,the differences were statistically significant (P<0.05).No serious adverse events occurred in either group.Conclusion:NMES combined with evidence-based nursing can improve muscle strength and function,reduce the incidence of ICU-AW,shorten the duration of mechanical ventilation and ICU treatment,and improve sleep quality in patients with ICU-AW without serious adverse events.
