目的:探讨股骨近端抗旋髓内钉(proximal femoral nail antirotation，PFNA)在老年股骨粗隆间骨折(intertrochanteric fracture of femur，IFF)中的应用效果。方法:选取 2021 年 3 月—2023 年 4 月于南京中医药大学沭阳附属医院治疗的 150 例老年IFF患者作为研究对象，采用随机数表法将患者分为对照组与观察组，各 75 例。对照组接受骨牵引治疗，观察组接受PFNA治疗。比较两组患者髋关节功能、疼痛程度、术后并发症发生情况及生活质量。结果:观察组术后3个月Harris髋关节评分(Harris hip score，HHS)评分高于对照组，视觉模拟评分法(visual analogue scale，VAS)评分低于对照组，差异有统计学意义(P<0。05);观察组并发症总发生率低于对照组，差异有统计学意义(P<0。05);观察组术后 3 个月认知、躯体、情绪、角色、社会功能评分高于对照组，差异有统计学意义(P<0。05)。结论:PFNA能改善老年IFF患者的髋关节功能，降低疼痛程度，减少术后并发症，提高生活质量。
Application Effect of Proximal Femoral Nail Antirotation in Elderly Patients with Intertrochanteric Fracture of Femur
Objective:To explore the application effect of proximal femoral nail antirotation(PFNA)in elderly patients with intertrochanteric fracture of femur(IFF).Method:A total of 150 elderly patients with IFF who treated in Shuyang Affiliated Hospital of Nanjing University of Chinese Medicine from March 2021 to April 2023 were selected as the research objects,the patients were divided into the control group and the observation group by random number table method,with 75 cases in each group.The control group was given bone traction treatment,and the observation group was given PFNA treatment.The hip joint function,pain degree,postoperative complications and quality of life were compared between two groups.Result:The Harris hip score(HHS)score of the observation group at 3 months after operation was higher than that of the control group,and the visual analogue scale(VAS)score was lower than that of the control group,the differences were statistically significant(P<0.05);the total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05);the scores of cognition,body,emotion,role and social function in the observation group at 3 months after operation were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:PFNA can improve the hip joint function of elderly patients with IFF,reduce the degree of pain,reduce postoperative complications and improve the quality of life.
