Relationship between Glycosylated Hemoglobin,Atherosclerosis Index of Plasma and Carotid Atherosclerosis in Elderly Patients with Acute Cerebral Infarction
Objective:To study the relationship between glycosylated hemoglobin(HbA1c),atherogenic index of plasma(AIP)and carotid atherosclerosis in elderly patients with acute cerebral infarction.Method:A total of 418 elderly patients with acute cerebral infarction admitted to Affiliated Hospital of Chifeng College from March 2022 to April 2023 were selected as the study group,and they were divided into normal group(n=135),mild group(n=145)and severe group(n=138)according to the severity of carotid atherosclerosis.In addition,224 healthy people received in our hospital during the same period were selected as the control group.The basic data of patients were collected,blood samples were taken and the following indexes were measured:triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),creatinine(Cre),uric acid(UA),remnant lipoprotein cholesterol(RLP-C),HbA1c and C-reactive protein(CRP).AIP,TG glucose index(TyG)and pulse pressure index(PPI)were calculated.The clinical parameters of the study group and the control group were compared.The levels of HbA1c,AIP and blood lipid parameters in patients with different severity of carotid atherosclerosis were analyzed,and analyze the correlation between HbA1c,AIP levels and carotid atherosclerosis.logistic regression analysis was used to analyze the influencing factors of carotid atherosclerosis in elderly patients with acute cerebral infarction,construct logistic regression model and analyze its predictive value.Result:There was no significant difference in gender,age,body mass index and smoking status between the study group and the control group(P>0.05).The levels of TC,TG,LDL-C,HbA1c,AIP,RLP-C,PPI,Cre,UA,CRP and TyG in study group were significantly higher than those in control group,HDL-C level was significantly lower than that in control group,the differences were statistically significant(P<0.05).The levels of TG,TC,HDL-C,LDL-C,HbA1c,AIP,RLP-C,PPI,Cre,UA,CRP and TyG in normal group,mild group and severe group were compared,and the differences were statistically significant(P<0.05).The levels of HbA1c and AIP were significantly positively correlated with severity of carotid atherosclerosis(P<0.05).logistic model analysis results showed that TG,TC,LDL-C,HbA1c,AIP,RLP-C,PPI and TyG were independent risk factors for carotid atherosclerosis in elderly patients with acute cerebral infarction,HDL-C was independent protective factor for carotid atherosclerosis in elderly patients with acute cerebral infarction(P<0.05).logistic regression model was constructed,receiver operating characteristic(ROC)curve results showed that the area under the curve of carotid atherosclerosis predicted by this regression model in elderly patients with acute cerebral infarction was 0.893,95%CI(0.831,0.954),sensitivity was 0.821,specificity was 0.819.Conclusion:HbA1c and AIP are positively correlated with carotid atherosclerosis in elderly patients with acute cerebral infarction,and the up-regulation of HbA1c and AIP levels is an independent risk factor for carotid atherosclerosis.
