Analysis of the Value of Luteinizing Hormone Base Value and Zinc Nutritional Status in the Diagnosis of Precocious Puberty in Girls
Objective:To explore the value of luteinizing hormone(LH)base value and zinc nutritional status in the diagnosis of precocious puberty in girls.Method:A total of 330 girls with precocious puberty admitted to Xiamen Huli District Maternal and Child Health Hospital from January 1,2021 to December 31,2023 were retrospectively selected.They were divided into 103 central precocious puberty group,227 peripheral precocious puberty group,and 100 healthy girls were selected as healthy control group at physical examination center.Baseline data,serum zinc and urinary zinc levels of the three groups of children were compared,and the correlation between LH base value and serum zinc level and sex hormone level in children with precocious puberty was analyzed.Receiver operating characteristic(ROC)curve of LH base value and serum zinc level combined in the diagnosis of precocious puberty in girls was drawn to evaluate the value of the combined diagnosis of LH base value and serum zinc level in the diagnosis of central precocious puberty in girls was analyzed.Result:In children with central and peripheral precocious puberty,body mass index(BMI),bone age,LH base value,follicle-stimulating hormone(FSH)basal value,uterine volume,ovarian volume,cervical length,insulin-like growth factor 1(IGF-1),prolactin(PRL),the proportion of estradiol(E2)≥5 pg/mL and the proportion of zinc supplementation history were higher than those of healthy control group,the differences were statistically significant(P<0.05).BMI,bone age,LH base value,FSH base value,uterine volume,ovarian volume,cervical length,IGF-1,PRL,the proportion of E2≥5 pg/mL ratio in central precocious puberty group was higher than that in peripheral precocious puberty group,the differences were statistically significant(P<0.05).The serum zinc level in the children with central and peripheral precocious puberty was higher than that in the healthy control group,and the serum zinc level in the children with central precocious puberty was higher than that in the peripheral precocious puberty group,the differences were statistically significant(P<0.05).The proportion of urinary zinc>1.00 mg/L in central precocious puberty group was the highest,and the difference was statistically significant(P<0.05).The basic value of LH and serum zinc level were positively correlated with the basic values of E2,PRL and FSH,the correlation between the basic value of LH and PRL was the highest(r=0.576,P<0.001),followed by the correlation between serum zinc and the basic value of FSH(r=0.513,P<0.001).The area under curve(AUC)of LH base value combined with serum zinc level for the diagnosis of central precocious puberty in girls were 0.852(95%CI:0.780,0.923),0.774(95%CI:0.697,0.850)and 0.879(95%CI:0.803,0.935),respectively,the sensitivity was 80.16%,77.25%,78.04%,and the specificity was 84.38%,65.41%,80.39%,respectively.Conclusion:LH base value and serum zinc level were higher in girls with precocious puberty.LH base value and zinc nutritional status were positively correlated with sex hormone levels in girls with precocious puberty.LH base value combined with zinc nutritional status was more accurate in diagnosing girls with precocious puberty.
