健侧卧位输尿管软镜联合负压清石治疗复杂型肾结石的临床应用研究

摘要
目的:分析健侧卧位输尿管软镜联合负压清石治疗复杂型肾结石的临床效果.方法:选取 2022 年 6 月—2023 年 12 月肇庆泓强康复医院收治的 80 例复杂型肾结石患者.根据随机单双数抽签法将其分为对照组(单数)和研究组(双数),各 40 例.研究组给予健侧卧位输尿管软镜碎石负压清石治疗,对照组给予常规输尿管软镜碎石术治疗.比较两组结石清除率、围手术期指标、并发症、术前及术后 24 h相关指标.结果:研究组结石清除率高于对照组,差异有统计学意义(P＜0.05).研究组手术时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P＜0.05).研究组术后并发症发生率低于对照组,差异有统计学意义(P＜0.05).术后 24 h,研究组白细胞计数、中性粒细胞百分比、血肌酐均低于对照组,血红蛋白高于对照组,差异有统计学意义(P＜0.05).结论:复杂型肾结石采取健侧卧位输尿管软镜联合负压清石可取得更理想的治疗效果,具有创伤低、术后恢复快、并发症发生率低等优势.
Abstract
Objective:To analyze the clinical effect of ureteral soft scope combined with negative pressure stone cleaning in healthy lateral position in the treatment of complex kidney stones.Method:A total of 80 patients with complex kidney stones admitted to Zhaoqing Hongqiang Rehabilitation Hospital from June 2022 to December 2023 were selected.They were divided into control group(singular number)and study group(even number)according to random odd-even number drawing method,with 40 cases in each group.The study group was treated with ureteral soft scope combined with negative pressure stone cleaning in healthy lateral position,and the control group was treated with conventional soft ureteral lithotripsy.The stone clearance rate,perioperative indexes,complications,preoperative and postoperative 24 h related indexes were compared between the two groups.Result:The stone clearance rate of the study group was higher than that of the control group,and the difference was statistically significant(P＜0.05).The operation time and hospital stay of the study group were shorter than those of the control group,and the amount of intraoperative blood loss was less than that of the control group,the differences were statistically significant(P＜0.05).The incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P＜0.05).After 24 h of surgery,the white blood cell count,neutrophil percentage and serum creatinine in the study group were lower than those in the control group,and hemoglobin was higher than those in the control group,the differences were statistically significant(P＜0.05).Conclusion:Ureteral soft scope combined with negative pressure stone cleaning in healthy lateral position can achieve better treatment effect for complex kidney stones,with advantages of low trauma,quick postoperative recovery and low complication rate.
