高通量血液透析治疗慢性肾脏病高磷血症患者的临床效果及对认知功能障碍的影响

目的:探讨高通量血液透析治疗慢性肾脏病高磷血症患者的临床效果及对认知功能障碍的影响。方法:选择2020 年 2 月—2022 年 12 月光泽县医院收治的 80 例慢性肾脏病高磷血症患者作为研究对象,按照随机数表法将患者分为对照组与观察组,各 40 例。对照组应用高磷血症基础治疗和常规血液透析治疗,观察组应用高磷血症基础治疗与高通量血液透析治疗。比较两组认知功能、肾功能、血磷、血钙、血甲状旁腺素、炎症因子及心脑血管事件发生情况。结果:观察组治疗后蒙特利尔认知评估量表(MoCA)评分、血钙水平高于对照组,认知功能障碍发生率、尿素氮(BUN)、血肌酐(Scr)、β2-微球蛋白(β2-MG)、血磷、血甲状旁腺素、C反应蛋白(CRP)及降钙素原(PCT)水平低于对照组,差异有统计学意义(P<0。05);两组心脑血管事件发生率比较,差异无统计学意义(P>0。05)。结论:高通量血液透析应用于慢性肾脏病高磷血症患者的临床治疗中,可改善认知功能、肾功能,调节机体血钙和血磷含量,改善机体炎症状态,且未增加心脑血管事件。
Clinical Effect of High-throughput Hemodialysis in the Treatment of Hyperphosphatemia in Chronic Kidney Disease and Its Impact on Cognitive Dysfunction
Objective:To investigate the clinical effect of high-throughput hemodialysis in the treatment of hyperphosphatemia in chronic kidney disease and its impact on cognitive dysfunction.Method:A total of 80 patients with hyperphosphatemia in chronic kidney disease who admitted to Guangze County Hospital from February 2020 to December 2022 were selected as the research objects,the patients were divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was treated with basic treatment of hyperphosphatemia and conventional hemodialysis,while the observation group was treated with basic treatment of hyperphosphatemia and high-throughput hemodialysis.The cognitive function,renal function,blood phosphorus,blood calcium,blood parathyroid hormone,inflammatory factors and cardiovascular and cerebrovascular events were compared between two groups.Result:The Montreal cognitive assessment(MoCA)score and serum calcium level in the observation group after treatment were higher than those in the control group,and the incidence of cognitive dysfunction,blood urea nitrogen(BUN),serum creatinine(Scr),β2-microglobulin(β2-MG),blood phosphorus,blood parathyroid hormone,C-reactive protein(CRP)and procalcitonin(PCT)levels were lower than those in the control group,and the differences were statistically significant(P<0.05);there was no significant difference in the incidence of cardiovascular and cerebrovascular events between two groups(P>0.05).Conclusion:The application of high-throughput hemodialysis in the clinical treatment of patients with hyperphosphatemia in chronic kidney disease can improve cognitive function,renal function,regulate body's blood calcium and blood phosphorus content,improve body's inflammatory state,and does not increase cardiovascular and cerebrovascular events.
