Analysis of Influencing Factors of Bone Cement Leakage During Pedicle Screw Fixation for Osteoporotic Spinal Fracture
Objective:To analyze the influencing factors of bone cement leakage during pedicle screw internal fixation surgery for osteoporotic spinal fractures.Method:The data of 98 patients with osteoporotic spinal fractures who underwent pedicle screw fixation in the hospital from June 2020 to June 2023 were retros pectively analyzed.Based on the occurrence bone cement leakage,the patients were divided into a bone cement leakage group and a normal bone cement group.The clinical characteristics of patients in the bone cement leakage group and the normal bone cement group were compared.Using logistic regression analysis to identify risk factors for bone cement leakage during pedicle screw fixation surgery in patients with osteoporotic spinal fractures.Result:Among 98 patients with osteoporotic spinal fractures treated with pedicle screw internal fixation,a total of 28 patients experienced cement leakage,with a leakage rate of 28.57%.There was no statistical significance in the comparism of gender,age and fracture degree between the bone cement leakage group and normal bone cement group(P>0.05).The timing of bone cement injection,the destruction of perivertebral wall,the shape of bone cement,the amount of bone cement injection and the degree of vertebral compression of≥50%in the bone cement leakage group were compared with those in the normal bone cement group,and differences were statistically significant(P<0.05).The results of logistic regression analysis showed that the degree of vertebral compression≥50%,the destruction of perivertebral wall,the amount of bone cement injection and the timing of bone cement injection were all independent risk factors for bone cement leakage during pedicle screw internal fixation in patients with osteoporotic spinal fractures(P<0.05).Conclusion:The degree of vertebral compression≥50%,the destruction of vertebral peripheral wall,the large amount of bone cement injection,and the time of bone cement injection in the Congee phase were all independent risk factors for bone cement leakage during pedicle screw internal fixation in patients with osteoporotic spinal fracture.
