Quality assessment and recommendations of clinical guidelines and consensus on hepatic encephalopathy
Objective:To provide a reference for the development of future guidelines and clinical practices on Hepatic Encephalopathy(HE),the methodological quality and reporting quality of HE guidelines were evaluated using AGREE Ⅱ and RIGHT tools.Additionally,the recommendations and evidence were summarized and compared.Methods:The databases including CNKI,PubMed,CBM,VIP,Wanfang,WHO,GIN,NGC,NICE SIGN and Medline databanse.The search period was from January 2003 to January 2023.Two liver disease researchers independently evaluated the methodological quality and reporting quality of the included guideline documents using AGREE Ⅱ and RIGHT,sorted out and compared the recommendations and evidence of the guideline documents formulated by evidence-based methods.Results:A total of 14 articles were included in this analysis,of which 9 were guidelines and 5 were expert consensus,including 2 Chinese articles and 12 English articles.Twelve studies were developed using evidence-based methods.Seven films were rated B and seven were rated C.The average scores of each domain of AGREE Ⅱ were scope and purpose(40.87%),stakeholder involvement(26.98%),rigor of development(23.96%),clarity of expression(47.22%),applicability(17.26%)and editorial independence(18.75%).The average scores of RIGHT were basic information(60.42%),background(40.85%),evidence(43.57%),recommendations(43.88%),review and quality assurance(16.96%),funding and conflict of interest statement and management(17.56%)and other aspects(38.10%).The key recommendations cover screening and diagnosis,treatment,nutrition management and prevention.Conclusion:The included HE guideline documents had overall low scores in methodological quality and reporting quality.In terms of methodological quality,participants,rigor of development,applicability,and editorial independence still need to be improved.In terms of reporting quality,review and quality assurance,funding and conflict of interest declarations and management areas need further reporting.In the future,radiotracer imaging and neurophysiology research can be improved to provide further reference for active diagnosis of HE,and to form a HE management model of"drug treatment,nutrition management and recurrence prevention".
