Diagnostic value of 7 miRNAs combined test for primary liver cancer
Objective:To analyze the clinical application value of combined detection of seven miRNAs in the diagnosis of primary liver cancer.Methods:Real-time PCR was used to detect the expression levels of seven miRNAs in 131 patients with primary liver cancer,metastatic liver cancer,and liver cirrhosis,and the positive rates of seven miRNAs in the three groups were compared.The diagnostic value of seven miRNAs in different pathological types of liver cancer was analyzed.To compare the positive rates of seven miRNAs in alpha-fetoprotein(AFP)negative and both AFP and abnormal prothrombin(PIVKA-Ⅱ)negative hepatocellular carcinoma(HCC)patients.The sensitivity and specificity of AFP,seven miRNAs and AFP+seven miRNAs in the diagnosis of HCC were analyzed,and the ROC curve was used to evaluate the diagnostic efficacy of the three.Results:The positive rate of seven miRNAs in primary liver cancer group was significantly higher than that in metastatic liver cancer group(x2=13.388,P<0.01)and cirrhosis group(x2=30.827,P<0.01).In different cell types of HCC,the positive rate of seven miRNAs in HCC group was significantly higher than that in ICC group,and the difference was statistically significant(HCC group vs ICC group x2=6.805,P<0.01).The positive rates of AFP,PIVKA-Ⅱ and seven miRNAs in HCC patients were 60.71%,64.28%and 75%,respectively.In HCC patients with negative AFP or negative AFP and PIVKA-Ⅱ,the positive rates of combined detection of seven miRNAs were 81.8%and 80.0%,respectively.The sensitivity of AFP,seven miRNAs and AFP+seven miRNAs in the diagnosis of HCC patients were 60.71%,75.00%and 89.29%,respectively,and the specificity were 81.33%,85.33%and 78.67%,respectively.The AUC(area under the ROC curve)of AFP,seven miRNAs and AFP+seven miRNAs in the diagnosis of HCC were 0.708,0.839 and 0.864,respectively.Conclusion:The seven miRNAs have high positive expression rates in HCC patients and AFP negative HCC.The combined detection of AFP and seven miRNAs can significantly improve the positive rate of HCC diagnosis.
