低管电压联合对比剂分段注射对门静脉CTA图像质量的影响研究

目的:探讨低管电压联合对比剂分段注射对门静脉CTA图像质量的影响.方法:选取2021年1月至2022年8月行上腹部增强+门静脉CTA检查的患者90例,随机分为三组,A组管电压120 kV,对比剂流速4 ml/s;B组管电压80 kV,对比剂流速4 ml/s;C组管电压80 kV,对比剂先以4 ml/s推注60%,再以2.5 ml/s推注40%.3组对比剂总量为1.2 ml/kg,生理盐水总量30 ml,流速4.0 ml/s.结果:C 组辐射剂量为(2.47±1.10)mSv,明显低于 A 组(3.28±1.46)mSv 和 B 组(3.59±1.57)mSv,可见 C组达到了辐射剂量最优,3组间两两比较差异有统计学意义(P<0.05).A组图像的CT值门静脉主干为(161.07±5.73)Hu,左支为(154.97±31.91)Hu,右支为(153.37±32.80)Hu,主观评分(3.43±0.82)分;B 组图像的 CT 值门静脉主干为(165.17±43.88)Hu,左支为(158.80±46.18)Hu,右支为(151.13±39.78)Hu,主观评分(3.63±1.10)分;C组图像的CT值门静脉主干为(265.94±198.88)Hu,左支为(258.59±50.95)Hu,右支为(254.81±57.51)Hu,主观评分(4.75±0.51)分,可见C组图像质量明显高于A、B两组(P<0.05).结论:低管电压联合分段注射在降低辐射剂量的同时提高了门静脉CTA的图像质量,值得临床推广应用.
Effect of segmental injection of low tube voltage combined with contrast medium on image quality of portal vein CT A
Objective:To investigate the effect of low kV combined with contrast injection on the image quality of portal vein CTA.Methods:A total of 90 patients who received upper abdominal enhanced portal vein CTA examination in our hospital from January 2021 to August 2022 were selected and randomly divided into ABC groups.Group A tube voltage was 120 kV,contrast agent flow rate 4 ml/s;Group B tube voltage is 80 kV,contrast agent flow rate was 4 ml/s;The tube voltage of group C is 80 kV,and the first section of contrast agent was injected with 60%at 4 ml/s,and then injected with 40%at 2.5 ml/s.The total amount of contrast agent in the three groups was 1.2 ml/kg,the total amount of normal saline was 30 ml,and the flow rate was 4.0 ml/s.Results:The radiation dose of 80 kV combined segmental injection group in Group C was(2.47±1.10)mSv,which was significantly lower than that of 120 kV one-stage injection group A(3.28±1.46)mSv and 80 kV one-stage injection group B(3.59±1.57)mSv.It can be seen that Group C reached the optimal radiation dose,and the difference between the three groups was statistically significant(P<0.05).In group A,the CT value of the main portal vein was(161.07±5.73)Hu,the CT value of the left branch was(154.97±31.91)Hu,the CT value of the right branch was(153.37±32.80)Hu,and the subjective score was 3.43±0.82;In group B,the CT value of the main portal vein was(165.17±43.88)Hu,the CT value of the left branch was(158.80±46.18)Hu,the CT value of the right branch was(151.13±39.78)Hu,and the subjective score was 3.63±1.10;The CT value of the main portal vein in group C was(265.94±198.88)Hu,the CT value of the left branch was(258.59±50.95)Hu,the CT value of the right branch was(254.81±57.51)Hu,and the subjective score was 4.75±0.51.It can be seen that the image quality of group C was significantly higher than that of group A and group B,and the difference was statistically significant(P<0.05).Conclusion:Low kV combined segmental injection can reduce the radiation dose and improve the image quality of portal vein CTA,which is worthy of clinical application.
