目的:研究miR-125b、miR-140 联合超声造影技术在肝占位性病变良恶性诊断中的效能及价值.方法:连续纳入并回顾分析2022 年6 月至2023 年6 月本院收治的肝脏占位性病变患者共125 例,根据病理活检结果,将患者分为良性组(n=65)及恶性组(n=60),并选取同期的健康体检者作为对照组(n=56),通过qRT-PCR实验检测血清miR-125b,miR-140 的相对表达水平,受试者进行超声造影检查.收集并分析受试者临床基本资料,绘制受试者工作特征(ROC)曲线分析血清miR-125b,miR-140 联合超声造影技术对肝脏占位性病变患者的诊断价值.结果:与对照组相比,肝占位性病变组患者的血清miR-125b、miR-140 水平显著降低(P<0.05),且与良性组相比,恶性组患者的血清miR-125b、miR-140 水平也显著降低(P<0.05).超声造影结果诊断发现良性病变患者68 例,恶性病变患者57 例,进一步分析超声造影特征发现,恶性病变组患者早增强、高增强和快消退比例显著高于良性病变组(P<0.05).临床基本资料分析发现:恶性组患者的丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)均显著高于良性组(P<0.05),并且两组患者的肿瘤直径及病理类型具有显著统计学差异(P<0.05),两组患者的年龄、性别无统计学差异(P>0.05).绘制ROC曲线发现血清miR-125b、miR-140 以及两者联合诊断的AUC分别为0.768、0.841、0.903,且血清miR-125b、miR-140 与超声造影联合诊断的特异性及预测准确率均高于miR-125b、miR-140、超声造影单独检测.结论:血清miR-125b、miR-140 联合超声造影在肝占位性病变良恶性诊断中具有较高的临床价值,可为临床应用提供参考.
Clinical application of contrast-enhanced ultrasound combined with serum miR-125b and miR-140 in the diagnosis of benign and malignant liver occupying lesions
Objective:To investigate the efficacy and value of miR-125b and miR-140 combined with contrast-enhanced ultrasound in the diagnosis of benign and malignant liver occupying lesions.Methods:A total of 125 patients with liver space occupying lesions admitted to our hospital from June 2022 to June 2023 was included and retrospectively analyze,according to the pathological biopsy results,they were grouped into a benign group(n=65)and a malignant group(n=60),and healthy individuals were collected as the control group(n=56),qRT-PCR experiment was applied to detect the relative expression levels of miR-125b and miR-140 in serum,and contrast-enhanced ultrasound examination was performed on patients.The basic clinical data of patients were collected and analyzed.The receiver operating characteristic(ROC)curve was plotted to analyze the diagnostic value of serum miR-125b,miR-140 combined with contrast-enhanced ultrasound in patients with benign and malignant liver occupying lesions.Results:Compared with the control group,the serum levels of miR-125b and miR-140 in patients in the liver space occupying lesions group were obviously reduced(P<0.05),and the serum levels of miR-125b and miR-140 in malignant group were also obviously lower than benign group(P<0.05).Ultrasonographic results diagnosed 68 patients with benign lesions and 57 patients with malignant lesions,and further analysis of the ultrasonographic features revealed that the proportion of patients in the malignant group with early enhancement,hyper-enhancement,and fast fading was significantly higher than that in the benign group(P<0.05).Clinical basic data analysis found that the Alanine aminotransferase(ALT),Aspartate aminotransferase(AST),and Alkaline phosphatase(ALP)levels in the malignant group were obviously higher than benign group(P<0.05),and there was an obvious statistical difference in tumor diameter and pathological type between the two groups(P<0.05),there was no statistical difference in age and gender between the two groups(P>0.05).Drawing ROC curves revealed that the AUC of serum miR-125b,miR-140,and their combined diagnosis was 0.768,0.841,and 0.903,respectively,the specificity and predictive accuracy of serum miR-125b,miR-140,and their combined diagnosis with contrast-enhanced ultrasound was higher than those of miR-125b,miR-140,and contrast-enhanced ultrasound alone.Conclusion:Serum miR-125b and miR-140 combined with ultrasonography have high clinical value in the diagnosis of benign and malignant liver occupying lesions.
